D-二聚体和降钙素原与C反应蛋白及血清淀粉样蛋白A检测在小儿呼吸道感染中的诊断价值

摘要 目的探讨D-二聚体(D-D)、降钙素原(PCT)、C反应蛋白(CRP)、血清淀粉样蛋白A(SAA)联合检测对小儿呼吸道感染的诊断价值。方法选取2020年1月至2021年12月于乐平市妇幼保健院儿科门诊就诊的98例呼吸道感染患儿作为研究对象,根据病原学检查结果分为细菌组(n=41)、非细菌组(n=57);另选取本院同期体检的35名健康儿童作为对照组。所有研究对象均进行血清D-D、PCT、CRP、SAA水平检测,比较3组血清D-D、PCT、CRP、SAA水平;采用Pearson线性相关分析法分析血清D-D与PCT、CRP、SAA的相关性;绘制ROC曲线分析血清D-D、PCT、CRP、SAA及联合检测对小儿呼吸道感染、感染类型的诊断价值。结果细菌组血清D-D、PCT、CRP、SAA水平均高于非细菌组与对照组,且非细菌组高于对照组,差异有统计学意义(P<0.05)。Pearson线性相关分析结果显示,患儿血清D-D与PCT、CRP、SAA水平均呈正相关(r>0,P<0.05)。血清D-D、PCT、CRP、SAA联合检测诊断小儿呼吸道感染的AUC为0.917,灵敏度为94.90%,特异度为88.57%,诊断价值最高;血清D-D、PCT、CRP、SAA联合检测鉴别细菌感染与非细菌感染的AUC为0.945,灵敏度为92.68%,特异度为92.98%,鉴别诊断价值最高。结论血清D-D、PCT、CRP、SAA联合检测对小儿呼吸道感染具有较高诊断价值,且能鉴别细菌感染与非细菌感染。 Objective To investigate the diagnostic value of D-dimer(D-D),procalcitonin(PCT),C-reactive protein(CRP)and serum amyloid A protein(SAA)in children with respiratory tract infection.Methods 98 children with respiratory tract infection treated in the Pediatric Outpatient department of Leping Maternal and Child Health Hospital from January 2020 to December 2021 were selected as the study subjects,and they were divided into the bacterial group(n=41)and the non-bacterial group(n=57)according to the results of etiological examination.Another 35 healthy children who underwent physical examination in our hospital during the same period were selected as the control group.Serum D-D,PCT,CRP and SAA levels were detected in all children,and the levels of serum D-D,PCT,CRP and SAA were compared among the three groups;the correlation between serum D-D and PCT,CRP and SAA was analyzed by Pearson linear correlation analysis;ROC was drawn to analyze the diagnostic value of serum D-D,PCT,CRP,SAA and combined detection in children with respiratory tract infection and infection types.Results The levels of serum DD,PCT,CRP and SAA in the bacterial group were higher than those in the non-bacterial group and the control group,and the non-bacterial group was higher than the control group,the differences were statistically significant(P<0.05).The results of Pearson linear correlation analysis showed that serum D-D was positively correlated with the levels of PCT,CRP and SAA(r>0,P<0.05).The AUC,sensitivity and specificity of the combined detection of serum D-D,PCT,CRP and SAA in the diagnosis of pediatric respiratory tract infection was 0.917,94.90%and 88.57%,respectively,and the diagnostic value was the highest.The AUC of,sensitivity and specificity of the combined detection serum D-D,PCT,CRP and SAA in differentiating bacterial infection from non-bacterial infection was 0.945,92.68%,92.98%,respectively,and the differential diagnostic value was the highest.Conclusion The combination of serum D-D,PCT,CRP and SAA has high diagnostic value i
