摘要
目的:探讨糖皮质激素在自身免疫性甲状腺疾病患者中的疗效观察及超声特点研究。方法:选择2016年6月至2019年6月期间我院收治的自身免疫性甲状腺疾病患者72例,随机分为对照组和观察组,每组36例。对照组给予常规对症治疗,观察组在对照组基础上采用糖皮质激素治疗,治疗后对患者效果进行评估,比较两组甲状腺素水平及甲状腺体积的变化、不良反应。结果:观察组治疗后2个月血清游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)的水平及甲状腺体积明显低于对照组,促甲状腺激素水平明显高于对照组(P<0.05);观察组治疗后不良反应的发生率低于对照组(P<0.05)。结论:在自身免疫性甲状腺疾病患者中合理应用糖皮质激素效果良好,经超声检查显示边界清晰、实质回声逐渐趋向均匀且甲状腺上动脉峰值流速亦趋流分布,有助于改善甲状腺激素水平及甲状腺腺体大小,且具有较好的安全性,值得推广应用。
Objective:To explore the efficacy of glucocorticoids in patients with autoimmune thyroid disease and the study of ultrasound characteristics.Methods:From June 2016 to June 2019,72 patients with autoimmune thyroid disease admitted to our hospital were randomly divided into a control group and an observation group,36 cases in each group.The control group was given conventional symptomatic treatment.The observation group was treated with glucocorticoids on the basis of the control group.After treatment,the patient's effect was evaluated,and the thyroid hormone levels and changes in thyroid body and adverse reactions were compared between the two groups.Results:The levels of free triiodothyronine,free thyroxine and thyroid volume in the observation group were significantly lower than those in the control group,and the levels of thyroid stimulating hormone were significantly higher than the control group(P<0.05);The incidence of adverse reactions after treatment in the group was lower than that in the control group(P<0.05).Conclusion:The rational application of glucocorticoids in patients with autoimmune thyroid disease has good effect.Ultrasound examination shows that the boundary is clear,the parenchymal echo gradually tends to be uniform,and the peak flow velocity of the superior thyroid artery also tends to be distributed.The glands are large in size and have good safety,which is worthy of popularization and application.
