摘要
目的建立我院50岁以上中老年人血浆D-二聚体参考值范围。方法选取2016年1月至2018年12月到昆明市延安医院进行健康体检的对象,其中20~49岁健康青年人135例为健康青年组。将50岁以上中老年人按年龄分为A组(50~59岁)134例、B组(60~69岁)142例、C组(70~79岁)155例、D组(≥80岁)143例。使用Sysmex CS5100全自动血凝仪采用免疫比浊法定量测定各研究对象的血浆D-二聚体含量,并对检测结果进行比较。采用95百分位数(P95)表示参考区间的单侧上限。结果中老年人中同一年龄组内不同性别间血浆D-二聚体浓度差异无统计学意义(P>0.05)。A组、B组、C组、D组的血浆D-二聚体水平与健康青年组相比,差异均有统计学意义(P<0.05)。A组、B组、C组、D组两两比较,差异均有统计学意义(P<0.008)。健康青年组、A组、B组、C组、D组的血浆D-二聚体参考范围分别为≤0.48 mg/L,≤0.64 mg/L,≤0.74 mg/L,≤0.76 mg/L,≤0.94 mg/L。结论本研究初步建立了我院中老年人血浆D-二聚体参考范围,有助于临床医生在中老年人群血栓性疾病的诊疗中根据年龄段选择相应的参考范围。
Objective To set up the reference range of plasma D-dimer for middle-aged and elderly people in Yan′an Hospital of Kunming.Methods A total of 574 cases over 50 years old in Yan′an Hospital of Kunming and 135 young cases of healthy control group were enrolled in this study.Persons aged over 50 years were divided into four groups group A(50-59 years old),group B(60-69 years old),group C(70-79 years old),and group D(≥80 years old).The level of plasma D-Dimer was tested by automatic coagulation analyzer Sysmex 5100,and was compared between the groups.The percentile 95(P95)was used to express the upper limit of one-sided reference range.Results There was no significant difference in the level of plasma D-Dimer between different genders in the same age group(P>0.05).The level of plasma D-dimer in group A,group B,group C and group D was significantly higher than that in healthy young group(P<0.05),and the difference of plasma D-Dimer among group A-D was statistically significant(P<0.008).The normal reference range of plasma D-dimer in healthy control group and group A,group B,group C,group D was≤0.48 mg/L,≤0.64 mg/L,≤0.74 mg/L,≤0.76 mg/L,≤0.94 mg/L,respectively.Conclusions This study establishes and evaluates the reference range of plasma D-Dimer level in the middle-aged and elderly people in our hospital,it is helpful for clinicians to select the corresponding reference range of D-Dimer according to age in the diagnosis and treatment of thrombotic diseases.
