摘要
目的探讨多发性大动脉炎颈动脉受累的超声表现并评估其临床价值。方法纳入本院2016年7月至2020年2月收治30例多发性大动脉炎颈动脉受累患者作为研究对象,纳入患者均进行彩色多普勒超声检查,检查结果与数字减影血管造影检查进行对比分析。结果30例患者血管壁改变情况主要表现颈总动脉受累与周边动脉受累,60条颈动脉受累中,颈总动脉受累46条、颈外动脉受累7条、颈内动脉受累7条,颈总动脉受累患者经诊断44条发生动脉壁IMT弥漫性增厚、2条局限性增厚。患者彩超扫描血管腔均发生改变。血管远端的血流速度降低,多呈低阻血流、静脉样的流速曲线。彩色多普勒超声扫描与数字减影血管造影的诊断一致性较高(P>0.05)。结论大动脉炎颈动脉受累患者可通过彩色多普勒超声检查进行诊断,其诊断准确性高,且能明确患者血管壁增厚程度及该处血流速度,对血管狭窄、阻塞或血栓形成可进行有效判断。
Objective To investigate the ultrasonic manifestations of carotid artery involvement in multiple arteritis and to evaluate its clinical value.Methods 30 patients with multiple arteritis and carotid artery involvement admitted to our hospital from July 2016 to February 2020 were included as the study subjects.All the included patients underwent color doppler ultrasonography,and the results were compared with digital subtraction angiography.Results 30 cases of patients with blood vessels changes mainly affected common carotid arteries and peripheral artery involvement,article 60 of the carotid artery involvement,common carotid artery involvement in article 46,involvement in article 7 external carotid artery,internal carotid artery involvement in article 7,common carotid artery affected by the diagnosis of 44 patients with diffuse thickening of the arterial wall IMT,thickening of two limitations.The vascular lumen of all patients were changed by color ultrasound scanning.The velocity of blood flow at the distal end of blood vessel decreased,and the flow curve was mostly of low obstruction and venous shape.The diagnostic consistency between color doppler ultrasonography and digital subtraction angiography was higher(P>0.05).Conclusion Patients with carotid artery involvement in arteritis can be diagnosed by color doppler ultrasonography,which has high diagnostic accuracy and can determine the degree of vascular wall thickening and blood flow velocity,and can effectively judge the stenosis,obstruction or thrombosis of blood vessels.
