摘要
目的探讨超声在儿童颈部多发性大动脉炎(TA)活动性评价的应用价值。方法选取郑州大学附属儿童医院2016年1月至2020年1月确诊的22例颈部TA患儿,分为活动期组10例和非活动期组12例,统计并比较两组患儿颈动脉受累管壁厚度及血流动力学的差异。治疗后活动期组患儿颈动脉受累管壁厚度与治疗前比较。结果22例TA患儿,共40支颈动脉受累,活动期组患儿23支,非活动期组患儿17支;其中24支管腔狭窄,活动期组患儿14支,非活动期组患儿10支;16支管腔无明显狭窄或局部扩张,活动期组患儿9支,非活动期组患儿7支。活动期组患儿颈动脉受累管壁厚度、管腔狭窄厚度和管腔无明显狭窄或局部扩张厚度均高于非活动期组患儿,差异有统计学意义(P<0.05)。两组患儿血流动力学比较,差异无统计学意义。治疗后,活动期组患儿颈动脉受累管壁厚度为(1.92±0.67)mm低于治疗前(2.87±1.03)mm,差异有统计学意义(P<0.05)。结论超声检测儿童TA颈动脉受累管壁厚度,对TA活动性具有一定提示意义。
Objective To investigate the application value of ultrasonography in the activity evaluation of children with multiple takayasu arteritis(TA).Method 22 patients with TA diagnosed in Children’s Hospital Affiliated to Zhengzhou University from January 2016 to January 2020 were selected and divided into active group(n=10)and inactive group(n=12).The thickness and hemodynamics of carotid artery in the two groups were compared.After treatment,the thickness of carotid artery in active group was compared with that before treatment.Result Among 22 TA patients,40 carotid arteries were involved,23 in the active group and 17 in the inactive group;among them,24 branch lumens were narrow,14 in the active group and 10 in the inactive group;no significant stenosis or local expansion was found in the 16 branch lumens,9 in the active group and 7 in the inactive group.The thickness of involved carotid artery wall,stenosis and no obvious stenosis or local dilation thickness in active group were higher than those in inactive group(P<0.05).There was no significant difference in hemodynamics between the two groups.After treatment,the wall thickness of carotid artery in active group was(1.92±0.67)mm,which was lower than(2.87±1.03)mm before treatment(P<0.05).Conclusion Ultrasound examination of the thickness of involved carotid artery wall in children with TA has a certain significance for TA activity.
