摘要
目的对糖化血红蛋白、胰岛β细胞功能在妊娠糖尿病诊断中的临床价值进行探讨。方法于2017年7月—2019年9月采集该院接受检查的妇女300例,按照糖耐量不同分为215例研究组与85例对照组,通过胰岛素释放试验与葡萄糖耐量试验对两组患者相关指标进行检测。按照血糖结果不同将研究组患者分为A组、B组、C组、D组。对比不同组别患者胰岛功能各项指标水平。结果相较于对照组,研究组HbA1c、空腹血糖、1 h-PG、2 h-PG、3 h-PG均水平更高(P<0.05)。相较于对照组,研究组空腹胰岛素、1 h胰岛素、2 h胰岛素、3 h胰岛素水平均更高(P<0.05)。相较于对照组,研究组AUCG、HOMA-IR、AUCI、水平更高,ISI及HOMA-β水平更低(P<0.05)。结论在妊娠糖尿病诊断中,HbA1c能够准确反映患者血糖水平变化情况,具有稳定性;妊娠糖尿病患者对于胰岛素抵抗力更高,但胰岛分泌功能呈现下降趋势;不同时间段血糖孕妇的胰岛素分泌功能会受损。
Objective To explore the clinical value of glycated hemoglobin andβ-cell function in the diagnosis of gestational diabetes.Methods A total of 300 women undergoing examination in the hospital from July 2017 to September 2019 were collected and divided into 215 study groups and 85 control groups according to different glucose tolerance.The relevant indexes of the two groups were detected by insulin release test and glucose tolerance test.Patients in the study group were divided into group A,group B,group C,and group D according to their blood glucose results.The levels of islet function in patients of different groups were compared.Results Compared with the control group,the levels of HbA1c,fasting blood glucose,1 h-PG,2 h-PG,and 3 h-PG were higher in the study group(P<0.05).Compared with the control group,the fasting insulin,1 h insulin,2 h insulin,and 3 h insulin levels in the study group were higher(P<0.05).Compared with the control group,the study group had higher levels of AUCG,HOMA-IR,AUCI,and lower levels of ISI and HOMA-β(P<0.05).Conclusion In the diagnosis of gestational diabetes,HbA1c can accurately reflect the changes of patients'blood glucose,with stability;patients with gestational diabetes have higher resistance to insulin,but the islet secretion function shows a downward trend;the insulin secretion function of pregnant women with blood glucose at different time periods will be impaired.
