摘要
目的:分析辅助生殖技术超促排卵对多囊卵巢综合征(PCOS)患者抗苗勒氏管激素(AMH)水平及妊娠率的影响。方法:90例PCOS患者作为观察组,同期90例输卵管性不孕患者作为对照组,2组均行辅助生殖技术超促排卵治疗1月;比较2组患者取卵日卵泡液及血清AMH水平,临床特征指标获卵率、正常受精率、临床妊娠率及继续妊娠率,比较观察组、对照组中妊娠与未妊娠患者取卵日子宫内膜厚度、优质胚胎比率、卵泡液及血清AMH水平。结果:观察组患者取卵日的卵泡液及血清AMH水平高于对照组,差异有统计学意义(P<0.05);观察组患者临床特征指标中的获卵率、卵子成熟率低于对照组,差异有统计学意义(P<0.05);观察组及对照组中妊娠患者优质胚胎比率、卵泡液及血清AMH水平高于未妊娠患者,差异有统计学意义(P<0.05)。结论:辅助生殖技术超促排卵可明显降低PCOS患者的卵泡液及血清AMH水平,但对PCOS及输卵管性不孕的妊娠率无显著影响。
Objective:To analyze the effect of assisted reproductive technology(ART)on polycystic ovary syndrome(PCOS)and tubal infertility on the level of anti-Mullerian hormone(AMH)and pregnancy rate.Methods:90 patients with PCOS were selected as the observation group and 90 patients with tubal infertility as the control group.Both groups were treated with ART.The AMH value of follicular fluid and serum on the day of ovum collection,the clinical characteristic indexes of the treatment cycle of ART and the difference of AMH level between pregnant and non-pregnant serum and follicular fluid were analyzed.Results:The AMH values of follicular fluid and serum on the day of ovum collection in the observation group were significantly higher than those in the control group,and the difference was statistically significant(P<0.05).The rate of oocyte acquisition and maturation in the observation group was significantly lower than in the control group,the difference was statistically significant(P<0.05).The level of AMH in follicular fluid and serum of pregnant women were significantly higher than those of non-pregnant women,the difference was statistically significant(P<0.05).The high-quality embryo rate,follicular fluid and serum AMH level of pregnant women were significantly higher than those of non-pregnant women,and the difference was statistically significant(P<0.05).Conclusion:The AMH level of PCOS and tubal infertility are significantly reduced by using ART,but there is no significant effect on the pregnancy rate of PCOS and tubal infertility.
