摘要
目的分析大柴胡汤加减联合针灸治疗热性惊厥的临床效果。方法选取医院2015年12月—2018年6月收治的热性惊厥患儿98例,根据治疗方法不同分为对照组和观察组。对照组患儿采用常规治疗联合针灸治疗,观察组患儿在对照组治疗基础上联合大柴胡汤加减治疗,分析两组患儿治疗后的临床疗效。结果观察组患儿总有效率为91.84%(45/49),对照组为73.47%(36/49),观察组患儿总有效率高于对照组,差异有统计学意义(P<0.05)。观察组患儿退热时间神志恢复时间及惊厥消失时间均短于对照组,差异有统计学意义(P<0.05)。治疗前,两组患儿神经因子、CRP、SF、MMP-9水平比较,差异无统计学意义(P>0.05)。治疗后,观察组患儿NSE、NPY、BDNF、CRP、MMP-9水平低于对照组,SF水平高于对照组,差异有统计学意义(P<0.05)。观察组患儿住院期间和出院3个月内复发率低于对照组,差异有统计学意义(χ~2=4.170,4.009;P=0.041,0.045)。结论大柴胡汤加减联合针灸治疗热性惊厥疗效显著,可改善患儿神经因子、SF、MMP-9水平,加快患儿恢复,降低复发率。
Objective To analyze the clinical effect of Dachaihu Decoction combined with acupuncture on febrile convulsion.Methods A total of 98 cases of febrile convulsion treated in our hospital from December 2015 to June 2018 were divided into control group and observation group according to different treatment methods.The children in the control group were treated with routine therapy combined with acupuncture and moxibustion,and those in the observation group were treated with Dachaihu Decoction on the basis of the control group.Results The total effective rate of the observation group was 91.84%(45/49)and that of the control group was 73.47%(36/49).The total effective rate of the observation group was higher than that of the control group(P<0.05).The time of antipyretic recovery and the disappearance of convulsion in the observation group were shorter than those in the control group(P<0.05).Before treatment,there was no significant difference in the level of MMP-9 between the two groups(P>0.05).After treatment,the levels of NSE,NPY,BDNF,CRP and MMP-9 in the observation group were lower than those in the control group,and the level of SF was higher than that in the control group.The difference was statistically significant(P<0.05).The recurrence rate in the observation group was significantly lower than that in the control group during hospitalization and within 3 months of discharge(χ~2=4.170,4.009;P=0.041,0.045).Conclusion Dachaihu Decoction combined with acupuncture and moxibustion has a significant effect on febrile convulsion,which can improve the levels of SF and MMP-9,accelerate recovery and reduce the recurrence rate.
