摘要
目的:探讨补肾通络法治疗血管性痴呆的临床疗效,并观察脑源性神经营养因子(BDNF)、淀粉蛋白前β位分解酶1(BACE1)和S100β蛋白水平的变化。方法:选取符合纳入标准的血管性痴呆患者80例,采用随机数字表分为对照组和观察组,每组各40例。对照组给予奥拉西坦联合针刺治疗;观察组在对照组的基础上联合补肾通络法治疗(肾虚血瘀者用补肾活血通络汤,肾虚痰浊者用补肾化痰通络汤)。两组均以4周为1个疗程,连续治疗3个疗程。比较两组治疗前后认知功能(MMSE)、神经功能缺损(NIHSS)、日常生活能力(ADL)和中医证候评分,同时比较BDNF、BACE1和S100β蛋白水平,并评价疗效。结果:观察组总有效率为90%(36/40),高于对照组72.5%(29/40),差异有统计学意义(P<0.05)。治疗后,观察组MMSE评分高于对照组,NIHSS评分、ADL评分和中医证候评分低于对照组(P<0.05)。观察组BDNF水平高于对照组,观察组BACE1、S100β蛋白水平低于对照组(P<0.05)。结论:补肾通络法治疗血管性痴呆疗效明确,能够显著改善患者痴呆症状,提高生活质量,并且能上调BDNF水平、下调BACE1和S100β蛋白表达。
Objective:To explore the effect of Bushen Tongluo therapy on vascular dementia,and to analyze the changes of BDNF,BACE1 and S100βprotein levels.Methods:80 patients with vascular dementia were randomly divided into the control group(n=40)and the observation group(n=40).The control group was treated with Oxiracetam combined with acupuncture;on which basis,the observation group was also treated with Bushen Tongluo therapy.Bushen Huoxue Tongluo Decoction was used for Kidney deficiency and blood stasis,Bushen Huatan Tongluo Decoction was used for Kidney deficiency and phlegm turbidity.Two groups were treated for 12 weeks.The scores of cognitive function(MMSE),neurological deficit(NIHSS),activity of daily living(ADL)and TCM symptom,as well as the levels of BDNF,BACE1 and S100βprotein were compared between the two groups before and after the treatment,and the curative effect was evaluated.Results:The total effective rate of the observation group was 90%(36/40),which was significantly higher than 72.5%(29/40)of the control group(P<0.05).The score of MMSE was higher,and the scores of NIHSS,ADL and TCM symptom were lower in the observation group that those in the control group(P<0.05).The level of BDNF was higher,and the levels of BACE1 and S100βprotein were lower in the observation group than those in the control group(P<0.05).Conclusion:Bushen Tongluo therapy is effective in treating vascular dementia.It can significantly improve the symptoms and quality of life of the patients with dementia.It can also up-regulate the level of BDNF and down-regulate the expressions of BACE1 and S100βprotein.
