血浆同型半胱氨酸水平对老年冠心病患者肾功能的预测研究 被引量:3

摘要 目的:分析老年冠心病患者用血浆同型半胱氨酸(Hcy)水平预测肾功能的价值。方法:2018年1-12月收治老年冠心病患者148例,分为高Hcy组88例与非高Hcy组60例。分析两组年龄、性别、合并糖尿病、空腹血糖(FBG)、总胆固醇(TC)、血尿酸(UA)、血肌酐(Cr)、丙氨酸氨基转移酶(ALT)、血浆脑钠肽(BNP)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、肾小球滤过率(eGFR)情况。结果:两组患者年龄、性别、合并糖尿病、FBG、TC、ALT、BNP、LDL-C、HDL-C比较,差异无统计学意义(P>0.05),但高Hcy组UA、Cr显著高于非高Hcy组,eGFR显著低于非高Hcy组,差异有统计学意义(P<0.05);老年冠心病患者血浆Hcy水平与血清UA、Cr呈正相关,而与eGFR呈负相关(P<0.05)。结论:老年冠心病患者血浆Hcy水平可能对肾功能有影响,尤其是高Hcy水平者,血清尿酸、血肌酐更高,而肾小球滤过率估计值则更低,即Hcy越高,则可能肾功能越差。 Objective:To analyze the value of plasma homocysteine(Hcy)level in predicting renal function in elderly patients with coronary heart disease.Methods:From January to December 2018,148 elderly patients with coronary heart disease were selected.They were divided into the high Hcy group(88 cases)and the non high Hcy group(60 cases).Age,gender,with diabetes mellitus,the fasting blood glucose(FBG),total cholesterol(TC),serum uric acid(UA),serum creatinine(Cr),alanine aminotransferase(ALT),plasma brain natriuretic peptide(BNP),low density lipoprotein(LDL-C),high density lipoprotein(HDL-C)and glomerular filtration rate(eGFR)were analyzed between the two groups.Results:There was no significant difference in age,gender,diabetes mellitus,FBG,TC,ALT,BNP,LDL-C and HDL-C between the two groups(P>0.05).However,UA and Cr in the high Hcy group were significantly higher than those in non high Hcy group,and eGFR was significantly lower than those in the non high Hcy group,the differences were statistically significant(P<0.05).The level of Hcy in elderly patients with coronary heart disease was positively correlated with UA and Cr,but negatively correlated with eGFR(P<0.05).Conclusion:The level of plasma Hcy in elderly patients with coronary heart disease may have an effect on renal function,especially in patients with high Hcy level,the serum uric acid and serum creatinine are higher,while the estimated value of glomerular filtration rate is lower,that is,the higher the level of Hcy,the worse the renal function may be.
