摘要
目的:探讨尿沉渣分析仪和尿干化学分析仪联合检测对尿路感染的诊断价值。方法:167例尿路感染患者均行尿沉渣分析和尿干化学法进行检查,分析细菌培养结果与亚硝酸盐阳性检出情况,分析两种方法单纯检测与联合检测的诊断效能。结果:①细菌培养结果:162例尿标本中培养结果阳性79例,阳性率为48.8%。其中大肠埃希菌52例,占65.8%;金黄色葡萄球菌7例,占8.9%;铜绿假单胞菌5例,占6.3%;肺炎克雷伯菌5例,占6.3%;粪肠球菌4例,占5.1%;链球菌4例,占5.1%;鲍曼不动杆菌1例,占12.7%;产气肠杆菌1例,占12.7%。②亚硝酸盐阳性检出率:尿沉渣分析仪检出亚硝酸盐阳性率为52.5%,尿干化学分析仪为50.0%,尿沉渣分析仪+尿干化学分析仪为54.9%,三种检测方法差异无统计学意义(P>0.05)。③诊断效能比较:尿沉渣分析仪的灵敏度为70.9%、特异性为78.5%、阳性预测值为71.5%、阴性预测值为62.4%、准确率为70.8%。尿干化学分析仪的分别为72.7%、76.3%、73.6%、56.3%、69.4%。尿沉渣分析仪+尿干化学分析仪分别为77.3%、84.6%、83.8%、73.1%、84.7%。结论:尿沉渣分析仪联合尿干化学分析仪能够大大提高尿路感染的诊断准确性,临床应用价值高。
Objective:To explore the diagnostic value of the combined detection of urine sediment analyzer and urine dry chemical analyzer for urinary tract infection.Methods:All 167 patients with urinary tract infection were examined by urine sediment analysis and urinary dry chemistry.The results of bacterial culture and positive detection of nitrite were analyzed.The diagnostic efficiency of the two methods of simple detection and combined detection was analyzed.Results:①Bacterial culture results:79 cases were positive in 162 urine samples,the positive rate was 48.8%.Among them,52 were Escherichia coli,accounting for 65.8%;7 were Staphylococcus aureus,accounting for 8.9%;5 were Pseudomonas aeruginosa,accounting for 6.3%;5 were Klebsiella pneumoniae,accounting for 6.3%;4 cases,accounting for 5.1%;4 cases of streptococci,accounting for 5.1%;1 case of Acinetobacter baumannii,accounting for 12.7%;1 case of Enterobacter aerogenes,accounting for 12.7%.②The positive detection rate of nitrite:the positive rate of nitrite detected by the urine sediment analyzer is 52.5%,the urine dry chemical analyzer is 50.0%,the urine sediment analyzer+urine dry chemical analyzer is 54.9%,three kinds of detection The difference in methods was not statistically significant(P>0.05).③Diagnostic efficacy comparison:the sensitivity of the urine sediment analyzer is 70.9%,the specificity is 78.5%,the positive predictive value is 71.5%,the negative predictive value is 62.4%,and the accuracy rate is 70.8%.The urine dry chemical analyzers were 72.7%,76.3%,73.6%,56.3%,and 69.4%.Urine sediment analyzer+urine dry chemical analyzer were 77.3%,84.6%,83.8%,73.1%,84.7%.Conclusion:Urine sediment analyzer combined with urinary dry chemical analyzer can greatly improve the diagnostic accuracy of urinary tract infection and has high clinical value.
