摘要
目的评价对桡骨远端骨折患者实施保守治疗、手术治疗的临床价值。方法选取2016年3月至2019年3月本院收治的桡骨远端骨折患者90例作为研究对象,按照治疗方案不同分为两组,各45例。对照组行保守治疗,观察组行手术治疗,观察两组临床疗效、桡骨短缩等情况。结果观察组经治疗后腕关节正常,活动无受限38例,临床总有效率为97.7%;对照组经治疗后腕关节功能有所恢复,痛感轻者10例,腕关节功能无明显变化、痛感剧烈、生活无法自理7例,临床总有效率为84.4%,两组比较差异有统计学意义(χ^2=4.939 0,P<0.05)。治疗后,观察组桡骨短缩幅度较小(2.65±0.23)mm,尺偏角幅度较小(18.23±1.33)°,掌倾角幅度较大(5.56±0.52)°,差异有统计学意义(t=31.562 6、14.896 8、19.609 2,P<0.05)。结论对桡骨远端骨折患者建议实施手术治疗方案。在提高临床疗效、缩短骨短缩幅度、尺偏角幅度以及改善掌倾角幅度方面效果显著。
Objective To evaluate the clinical value of conservative treatment and surgical treatment for patients with distal radius fractures.Methods 90 patients with distal radius fractures admitted to our hospital from March 2016 to March 2019 were selected as study subjects. They were divided into two groups according to different treatment regimen, with 45 cases in each group. The control group was treated conservatively.The observation group was treated surgical treatment and observed the clinical efficacy and shortening of the tibia. Results In the observation group, normal wrist joint and unrestricted movement was 38 cases, and the clinical efficacy was 97.7%. In the control group, wrist function recovered after treatment, with 10 cases of mild pain, no significant changes in wrist function, severe pain, and unable to take care of themselves was 7 cases,with clinical efficacy of 84.4%. The difference was statistically significant between the two groups(χ^2=4.939 0, P<0.05). After treatment, the radius shortening range in the observation group was smaller(2.65±0.23) mm, the ulnar angle was smaller(18.23±1.33)°, and the palmar dip angle was larger(5.56±0.52)°. The difference was statistically significant(t=31.562 6, 14.896 8, 19.609 2, P<0.05). Conclusion The treatment of patients with distal radius fractures is recommended to adopt surgical treatment plan. It has significant effect in improving the clinical effect, shortening the amplitude of bone shortening, ulnar deflection and palmar inclination.
