摘要
目的探讨社区健康管理对老年糖尿病患者血糖、血压、血脂的控制效果。方法选取2016年5月-2017年5月作者所在社区医院诊治的200例老年糖尿病患者,对所有患者进行1年的健康管理,观察比较患者健康管理前后血糖、血压和血脂的控制情况。结果对患者健康管理前后的各项指标进行比较,健康管理前SBP(139.5±18.3)mmHg、DBP (94.2±5.5)mmHg、FPG (7.8±2.7)mmol/L、BMI (26.9±2.1)kg/m^2、TG (1.9±0.5)mmol/L、TC (5.4±0.3)mmol/L;健康管理后,SBP(130.1±10.8)mmHg、DBP(79.7±5.2)mmHg、FPG(7.0±2.4)mmol/L、BMI(23.1±1.6)kg/m^2、TG (1.6±0.2)mmol/L、TC(4.5±0.6)mmol/L;各指标前后对比均差异有统计学意义(tSBP=4.285,tDBP=4.913,tFPG=4.597,tBMI=4.048,tTG=4.253,tTC=4.982,P<0.05)。结论对社区老年糖尿病患者进行健康管理,其血糖、血压、血脂控制效果显著,有助于降低并发症发生风险,值得临床推广运用。
Objective To explore the effect of community health management on the control of blood glucose, blood pressure and blood lipid in elderly patients with diabetes. Methods From May 2016 to May 2017, 200 elderly diabetic patients diagnosed and treated in the community hospital of the author were selected. All patients were managed for 1 year, and the control of blood glucose, blood pressure and blood lipids before and after health management were observed and compared.Results The various indicators of patients before and after health management were compared. Before health management,SBP(139.5±18.3)mmHg, DBP(94.2±5.5)mmHg, FPG(7.8±2.7)mmol/L, and BMI(26.9±2.1)kg/m^2, TG(1.9±0.5)mmol/L, TC(5.4±0.3)mmol/L;after health management, SBP(130.1±10.8)mmHg, DBP(79.7±5.2)mmHg, FPG(7.0±2.4)mmol/L, BMI(23.1 ±1.6)kg/m^2, TG(1.6 ±0.2)mmol/L, TC(4.5 ±0.6)mmol/L;the differences between the indexes before and after the comparison were statistically significant(tSBP=4.285, tDBP=4.913, tFPG=4.597, tBMI=4.048, tTG=4.253, tTC=4.982, P <0.05).Conclusion The health management of elderly diabetic patients in the community has significant blood glucose, blood pressure, and lipid control effects, which helps reduce the risk of complications and is worthy of clinical promotion and application.
