摘要
笔者基于“血不利则为水”理论,结合多年来运用该理论治疗冠心病心力衰竭的临床体会和用药心得,认为“血不利则为水”是中医治疗冠心病心力衰竭的重要理论依据,活血利水法应为中医治疗冠心病心力衰竭的根本方法。并根据“血不利则为水”理论及导致血不利的原因对冠心病心力衰竭进行分型论治。其中,阳气虚损、血瘀水泛证,宜用春泽汤加丹参、鬼箭羽、桃仁、葶苈子等活血利水;宗气亏虚、血瘀水停证,予参芪丹参饮合升陷汤加减;气郁痰热、血瘀水停证,予柴胡丹陷汤加减;气虚痰阻、血瘀水停证,宜用醒脾汤化裁;气阴亏虚、痰瘀水停证,用生脉陷胸汤加利水之品。以期对后世学者运用中医药治疗冠心病心力衰竭有所裨益。
The article is based on the famous theory of‘unsmooth blood circulation results in water retention’,combined with his own clinical experience and medication experience in the treatment of heart failure caused by coronary heart disease for many years.It is believed that‘unsmooth blood circulation results in water retention’is an important theoretical basis for the treatment of heart failure caused by coronary heart disease with traditional Chinese medicine,and activating blood circulation for diuresis should be the fundamental method for treating heart failure caused by coronary heart disease in traditional Chinese medicine.According to the theory of‘unsmooth blood circulation results in water retention’and the causes of unsmooth blood circulation,it can be used to classify heart failure caused by coronary heart disease.Among them for yang qi deficiency and syndrome of water overflowing due to blood stasis,it is appropriate to use Chunze Decoction added salvia miltiorrhiza,winged euonymus twig,peach seed,semen lepidii and other drugs to promote blood circulation and excreting pathogenic water.For zong qi deficiency and blood stasis causing water retention syndrome,it is given Shenqi Danshen Drink combined with Shengxian Decoction.For qi stagnation,phlegm heat and blood stasis causing water retention syndrome,it is given modified Chaihu Danxian Decoction.For qi deficiency,phlegm resistance and blood stasis causing water retention syndrome,it is given modified Xingpi Decoction.For qi and yin deficiency,phlegm and blood stasis causing water retention syndrome,it is given Shengmai Xianxiong Decoction added the Chinese materia medica of inducing diuresis.It is hoped that scholars in later generations will benefit from the use of Chinese medicine in the treatment of heart failure caused by coronary heart disease.
