摘要
【目的】观察基于脑-肾-督脉轴理论的督脉穴位埋线配合康复训练对痉挛型脑瘫患儿肌张力的影响。【方法】将90例痉挛型脑瘫患儿随机分为对照组、针刺组、埋线组,每组各30例。对照组采用神经发育学疗法(Bobath疗法)和Vojta疗法(又称Vojta诱导疗法)相结合为主的运动疗法进行康复训练,针刺组在接受康复训练的基础上配合督脉针刺治疗,埋线组在接受康复训练的基础上进行督脉穴位埋线治疗。分别于治疗前和治疗3个月后对3组患儿进行粗大运动功能测试量表(GMFM)评分及下肢肌肉痉挛程度的改良Tardieu量表(MTS)评估,并进行安全性评价。【结果】(1)治疗后,3组患儿的GMFM评分均较治疗前提高,差异均有统计学意义(P<0.05);但治疗后各组间比较,差异均无统计学意义(P>0.05)。(2)治疗后,3组患儿内收角、腘窝角及足背屈角的MTS关节角度均较治疗前增大,差异均有统计学意义(P<0.05);且治疗后针刺组和埋线组内收角、腘窝角及足背屈角的MTS关节角度均明显大于对照组,差异均有统计学意义(P<0.05或P<0.01);而针刺组与埋线组比较,差异均无统计学意义(P>0.05)。(3)安全性评价:治疗期间,患儿均未出现晕针、局部感染以及因操作不当而造成的脏器损伤等不良反应。【结论】基于脑-肾-督脉轴理论的督脉穴位埋线配合康复训练治疗痉挛型脑瘫疗效确切,能有效缓解患儿下肢肌肉痉挛,改善关节活动度,降低肌张力,对提高粗大运动功能也有一定作用。
Objective To observe the effect of Governor Vessel acupoint catgut embedding,a therapy established on the basis of brain-kidney-Governor Vessel axis theory,combined with rehabilitation training on muscle tension in children with spastic cerebral palsy.Methods Ninety children with spastic cerebral palsy were randomly divided into control group(n=30),acupuncture group(n=30)and catgut embedding group(n=30).The control group was mainly treated with the combination of Bobath therapy and Vojta therapy for the rehabilitation training.In addition to the rehabilitation training,the acupuncture group was treated with the acupuncture at Governor Vessel,while the catgut embedding group received Governor Vessel acupoint catgut embedding therapy.Before treatment and after 3 months of treatment,the gross motor function of the children in the three groups was evaluated by Gross Motor Function Measure(GMFM)scale,and the spasm degree of lower limb muscle was determined by Modified Tardieu Scale(MTS).Results(1)After the treatment,GMFM scores of the three groups were significantly increased in comparison with those before treatment(P<0.05),but the difference among the three groups after treatment was insignificant(P>0.05).(2)After the treatment,MTS joint angle of adduction angle,popliteal fossa angle,and dorsum flexion angle in the three groups was increased after the treatment in comparison with those before treatment(P<0.05),and the joint angle in the acupuncture group and catgut embedding group was higher than that in the control group,the difference being statistically significant(P<0.05 or P<0.01).However,the difference of the MTS joint angle between the acupuncture group and catgut embedding group was not statistically significant(P>0.05).(3)During the treatment,no faint during needling,local infection,or organ injury due to mistake operation in the three groups occurred.Conclusion The Governor Vessel acupoint catgut embedding based on brain-kidney-Governor Vessel axis theory plus rehabilitation training exerts certain effect
