表观扩散系数直方图在评估无灶性脑瘫患儿脑损伤的应用价值 被引量:4

摘要 目的:评价表观弥散系数(ADC)直方图定量分析方法在评估无灶性脑瘫(CP)患儿脑损伤严重程度的应用价值。方法:前瞻性选取在医院就诊的36例CP患儿,并将其纳入CP组;另选35名正常儿童的MR资料,并将其纳入对照组。使用MRIcron和ImageJ软件获得两组内囊后肢层面ADC直方图及各参数,对脑瘫组患儿的粗大运动功能测试量表(GMFM)评分与ADC直方图各参数值进行相关性分析,并分析两组直方图各参数值的差异。结果:CP组患儿的ADC第5百分位数(ADC 5th)至第95百分位数(ADC 95th)值均高于对照组,差异有统计学意义;ADC平均值(ADCmean)和ADC峰值(ADCpeak)CP组高于对照组,差异有统计学意义(t=8.27,t=8.28;P<0.05);最小ADC(ADCmin)两组间差异无统计学意义(t=0.68,P>0.49),两组直方图图形的偏度、峰度和熵间的差异有统计学意义(t=-6.82,t=-5.17,t=8.18;P<0.01);CP组患儿的ADC 5th至ADC 95th、ADCmean、ADCpeak与GMFM评分均具有相关性,ADCmin与GMFM评分无相关性(r=0.03,P>0.05)。结论:ADC直方图可以快速客观地提供脑组织更多的弥散信息,是识别CP患儿脑改变的一种快速的定量分析方法。 Objective: To evaluate the application value of quantitative analysis method of apparent diffusion coefficient(ADC) histogram in assessing the severity of brain injury of children with no focal cerebral palsy(CP). Methods: 36 children with no focal CP were perspective selected and they were divided into CP group. And the magnetic resonance(MR) data of 35 normal children were divided into control group. The MRIcron software and ImageJ software were used to obtain ADC histogram and each parameter of posterior limb of internal capsule of two groups. And the correlation analysis between the scores of gross motor function measure(GMFM) and each parameter of ADC histogram for children of CP group was implemented. And the difference of each parameter of histogram between two groups was analyzed further. Results: From P5 to P95 of ADC values, those of CP group were significantly higher than those of control group, respectively, and the mean of ADC and peak value of ADC of CP group were significantly higher than those of control group(t=8.27, t=8.28, P<0.05), respectively. The difference of minimum value of ADC(ADCmin) between two groups was no significant(t=0.68, P>0.49). The differences of skewness, kurtosis and entropy of histogram between two groups were significant(t=-6.82, t=-5.17, t=8.18, P<0.01). In CP group, the ADC values from P5 to P95, ADCmean and ADCpeak was correlated with GMFM score, respectively. While ADCmin was not correlated with that(r=0.03, P>0.05). Conclusion: ADC histogram can rapidly and objectively provide more diffuse information of brain tissue, and it is a fast quantitative analysis method to identify cerebral changes in children with CP.
