摘要
目的探讨GnRH-a长方案体外受精助孕周期HCG日不同孕酮水平的治疗效果。方法回顾性分析于2015年1月至2017年12月在广西壮族自治区妇幼保健院生殖中心实施常规体外受精的助孕周期的患者的临床资料,根据HCG日孕酮水平将其分为五组,其中P≤1.59nmol/L患者设为A组(n=78)、1.59nmol/L<P≤3.18nmol/L的患者设为B组(n=482)、3.18nmol/L<P≤4.77nmol/L患者计设为C组(n=345)、4.77nmol/L<P≤6.36nmol/L的患者设为D组(n=67例)、P>6.36nmol/L患者设为E组(n=22),比较各组之间患者一般资料及治疗效果。结果五组患者间的平均获卵数比较,其差异具有统计学意义(P<0.05),当P≤4.77nmol/L时,平均获卵数A组<B组<C组,其差异具有统计学意义(P<0.05);当P>4.77nmol/L时,平均获卵数D组<E组,其差异无统计学意义(均P>0.05)。五组患者间的正常受精率比较,其差异具有统计学意义(P<0.05)。五组患者间的D3平均可利用胚胎数比较,其差异具有统计学意义(P<0.05),其中A组<B组<C组<D组<E组,差异均有统计学意义(均P<0.05)。五组患者间的移植周期妊娠率对比,其差异无统计学意义(P>0.05)。五组患者间的周期冷冻率对比,A组<B组<C组<D组<E组,其差异均具有统计学意义(P<0.05)。五组患者间的累积1次胚胎移植妊娠率的比较,其差异无统计学意义(P>0.05)。结论在GnRH-a长方案体外受精助孕周期中,HCG日血清孕酮水平在3.18~4.77nmol/L范围时,有比较理想的获卵数、可利用胚胎数和周期冷冻率,移植周期妊娠率也较为稳定,而HCG日血清孕酮水平>4.77nmol/L时获卵数、可利用胚胎数不受孕酮升高的影响,有较高的周期冷冻率,但累计1次胚胎移植的妊娠率无影响。
Objective To explore the therapeutic outcome of different progesterone levels on the HCG day of in vitro fertilization(IVF)during the IVF cycle of GnRH-a.Methods Retrospective analysis of the clinical data of patients conducted IVF in our center from January 1,2015 to December 31,2017 was performed.All the patients were divided into 5 groups:78 cases in group A(P≤1.59 nmol/L),482 cases in group B(1.59 nmol/L6.36 nmol/L).The basic information and treatment effect was compared between different groups.Results There was significant difference in the average number of eggs obtained between the five groups(P<0.05).When P≤4.77 nmol/L,the average number of eggs obtained in group A<group B<group C,with statistically significant differences(P<0.05).When P>4.77 nmol/L,the average number of eggs obtained in group D was less than that in group E,but there was no significant difference(P>0.05).There was significant difference in the normal fertilization rate between the five groups(all P<0.05).The average number of D3 available embryos among the five groups was significantly different(P<0.05),group A<group B<group C<group D<group E,with statistically significant differences(all P<0.05).There was no significant difference in pregnancy rate between the five groups during transplantation cycle(P>0.05).There were significant differences in the freezing rate among the 5 groups,group A<group B<group C<group D<group E,with statistically significant differences(P<0.05).There was no significant difference in the cumulative pregnancy rate of one embryo transfer between the five groups(P>0.05).Conclusions In the in vitro fertilization cycle of GnRH-a long-term fertilization,there are ideal number of eggs obtained,number of available embryos and cycle freezing rate and pregnancy rate of transplant cycle when the HCG daily serum progesterone level is in the range of 3.18 to 4.77 nmol/L.When the serum progesterone level is over 4.77 nmol/L,the number of available embryos is not affected by the increase of progesterone,with high cycle f
