摘要
目的:探讨前交叉韧带(ACL)重建术后移植物的韧带化过程及其影响因素。方法:选取2013年1月至12月期间的膝关节镜下ACL重建手术患者,记录患者的性别、年龄、体重、身高、体重指数(BMI)、关节松弛度评分(Beighton评分)、ACL断裂病程时长和半月板处理情况。术后6月、1年、2年和4年随访时进行膝关节稳定性检查,并通过核磁检查测量韧带移植物近端、中部和远端的信号强度,分别计算信噪比(SNQ),用以评估重建韧带的韧带化情况。对全部数据进行统计分析,探讨韧带移植物的韧带化情况及其影响因素,以及与膝关节稳定性的相关性。结果:共计71例ACL重建患者入选研究,结果显示韧带移植物近端、中部和远端的SNQ值均随时间延长逐渐减小,韧带中部的SNQ值在术后2年、韧带近端和远端的SNQ值在术后4年呈现显著性降低(P<0.05)。性别、年龄、体重、身高、BMI、Beighton评分、ACL断裂时长和半月板处理情况等因素对韧带移植物的SNQ数值均无显著性影响(P>0.05),SNQ数值与随访时膝关节稳定性无显著相关性(P>0.05)。结论:ACL重建术后韧带移植物的韧带化情况随时间延长而逐步改善,韧带中部于重建术后2年、韧带近端和远端于重建术后4年,移植物的韧带化情况得到显著改善。
Objective To observe the ligamentization process of autogenous hamstring grafts after ante rior cruciate ligament(ACL)reconstruction and explore its potential influencing factors.Methods Pa tients undergoing the ACL reconstruction between January and December 2013 were recorded demo graphic data including their gender,age,weight,height,body mass index(BMI),joint relaxation score(Beighton score),time from ACL injury to reconstruction,and concomitant meniscal surgery.Their knee stability was examined at 6 months,1,2 and 4 years after the operation.The signal intensity of the proximal,middle and distal part of the ACL grafts was measured using MRI,and the signal to noise quotient(SNQ)was calculated to assess the graft maturity.The data were analyzed statistically to explore the ligamentization process of grafts and its potential influencing factors,as well as their corre lation with knee joint stability.Results A total of 71 patients with ACL reconstruction were enrolled in this study.The results showed that the SNQ value of the proximal,middle and distal ligament grafts decreased gradually with time,and significant differences were observed in the SNQ value of the mid dle part at 2 years,proximal and distal part at 4 years after the operation(P<0.05).The average SNQ values of grafts were not significantly affected by the gender,age,body weight,height,BMI,Beigh ton score,time from ACL injury to reconstruction,or concomitant meniscal surgery(P>0.05).There was no significant correlation between SNQ values and knee joint stability(P>0.05).Conclusion The ligamentization of autogenous hamstring grafts after ACL reconstruction improves gradually over time,and significant differences were observed in the SNQ value of the middle part at 2 years and of the proximal and distal part at 4 years after the operation.
