摘要
目的探讨关节镜下解剖位单束(ASB)重建与过顶位单束(OSB)重建联合玻璃酸钠腔内注射前交叉韧带(ACL)的疗效分析。方法选取2016年12月至2018年12月湖北省麻城市人民医院收治的90例单膝前交叉韧带损伤患者作为研究对象,根据随机数字表法分为对照组与研究组,每组各45例。对照组采取关节镜下OSB治疗,研究组采取关节镜下ASB治疗,比较两组患者国际膝关节评分委员会(IKDC)、膝关节Lysholm评分表、膝关节活动度(KT-1000)等指标。结果治疗后研究组患者的IKDC、Lysholm评分高于对照组(P<0.05);两组患者治疗前IKDC、Lysholm评分比较,差异无统计学意义(P>0.05);治疗后研究组患者的KT-1000评分明显高于对照组(P<0.05);两组患者治疗前KT-1000评分比较,差异无统计学意义(P>0.05);治疗前、后两组患者的Lachman阳性率比较,差异无统计学意义(P>0.05);治疗后研究组患者轴移试验阳性率为4.44%,低于对照组的17.78%(P<0.05);两组患者治疗前轴移试验阳性率比较,差异无统计学意义(P>0.05)。结论对前交叉韧带损伤患者实施关节镜下ASB治疗,相较于传统OSB术式,明显提升患者膝关节功能恢复效果,同时可有效加强患者膝关节旋转稳定性,临床价值显著。
Objective To investigate the efficacy of arthroscopic anatomical single-bundle(ASB)reconstruction and over-the-top single-bundle(OSB)reconstruction combined with sodium hyaluronate intracavitary injection of anterior cruciate ligament(ACL).Methods Ninety patients with single knee anterior cruciate ligament injury admitted to Macheng People′s Hospital in Hubei Province from December 2016 to December 2018 were selected as the study subjects.According to the random number table,they were divided into the control group and the study group,with 45 cases in each group.The control group was treated with arthroscopic OSB,and the study group was treated with arthroscopic ASB.The International Knee Documentation Committee(IKDC),Lysholm knee score table,and knee range of motion(KT-1000)were compared between the two groups.Results After treatment,IKDC and Lysholm scores in the study group were significantly higher than those in the control group(P<0.05).There was no significant difference in IKDC and Lysholm scores before treatment between the two groups(P>0.05).After treatment,the KT-1000 score in the study group was significantly higher than that in the control group(P<0.05).There was no significant difference in KT-1000 score before treatment between the two groups(P>0.05);There was no significant difference in Lachman positive rate before and after treatment between the two groups(P>0.05).After treatment,the positive rate of the pivot shift test in the study group was 4.44%,lower than 17.78%in the control group(P<0.05).There was no significant difference in the positive rate of pivot shift test before treatment between the two groups(P>0.05).Conclusion Compared with traditional OSB,arthroscopic ASB can significantly improve the recovery of knee joint function and effectively strengthen the rotational stability of knee joint in patients with anterior cruciate ligament injury,with significant clinical value.
