摘要
目的:分析磁共振成像(MRI)对青少年膝关节半月板损伤的特点和诊断价值,基于关节镜为金标准。方法:回顾性分析2016年4月—2018年8月新疆医科大学第六附属医院收治的126例青少年膝关节损伤病例。将252个半月板MRI诊断结论与关节镜检查结果对照,统计分析半月板损伤和半月板Ⅲ级损伤的敏感度、特异度、准确度和Kappa值,并分别计算0级和Ⅰ级、Ⅱ级、Ⅲ级损伤半月板的MRI表现准确度等指标。结果:对于青少年半月板损伤,MRI与关节镜检查结果比较,差异有统计学意义(P<0.05);MRI检查的特异度为53.6%,敏感度为96.5%,准确度为73.0%,阴性预测值为94.9%,阳性预测值为63.2%,Kappa值为0.479;Ⅲ级损伤的MRI诊断与关节镜检查差异无统计学意义(P>0.05),特异度、敏感度、准确度、阴性预测值、阳性预测值、Kappa值分别为95.1%、85.7%、93.3%、96.5%、80.8%、0.79;MRI诊断青少年半月板损伤Ⅲ级准确度为93.3%、Ⅱ级准确度为84.52%、0级和Ⅰ级准确度为84.13%。结论:对青少年半月板Ⅲ级损伤,MRI检查不仅准确度高,而且属于无创检查。MRI检查对于术前或治疗前评估具有较高的指导意义,但对于Ⅰ级和Ⅱ级损伤有待进一步提高诊断价值。
Objective With arthroscopy as the gold standard,the characteristics and diagnostic value of magnetic resonance imaging(MRI)for knee meniscus injury in adolescents were analyzed and evaluated.Methods A retrospective analysis was made of 126 cases of knee joint injury in adolescents admitted to the Sixth Affiliated Hospital of Xinjiang Medical University from April 2016 to August 2018.The sensitivity,specificity,coincidence rate and Kappa value of 252 meniscuses were examined by MRI and compared with arthroscopy.The coincidence rates of grade 0,gradeⅠ,gradeⅡand gradeⅢmeniscus injuries were calculated.Results There was significant difference between MRI and arthroscopy in adolescent meniscus injury(P<0.05).The specificity was 53.6%,the sensitivity was 96.5%,the coincidence rate was 73.0%,the negative predictive value was 94.9%,the positive predictive value was 63.2%,and the Kappa value was 0.479.There was no statistical difference between MRI diagnosis and arthroscopy examination of gradeⅢmeniscus injury in adolescents.The specificity,sensitivity,coincidence rate,negative predictive value,positive predictive value and Kappa value were 95.1%,85.7%,93.3%,96.5%,80.8%and 0.79,respectively.The coincidence rates of gradeⅢ,gradeⅡand grade 0,gradeⅠin the diagnosis of meniscus injury in adolescents were 93.3%,84.52%and 84.13%respectively.Conclusion For gradeⅢmeniscus injuries in adolescents,MRI examination is not only a high compliance rate,but also a non-invasive examination,it has high guiding significance.But for grade I andⅡinjuries,the diagnostic value needs to be further enhanced.
