摘要
目的:提高临床合理用药水平,提高药师处方审核的效率与质量,构建医院审方中心平台,探讨在临床合理用药中的效果。方法:引入审方中心系统,通过建立和维护系统的知识库,协助药师进行处方/医嘱审核。结果:2018年6月—2019年8月,住院医嘱通过系统8级直接拦截的医嘱为1833组,药师人工复审核医嘱368880组,干预医嘱1383组。结论:利用审方中心平台,实行医嘱的前置审核,可有效及时拦截不合理处方,降低临床用药差错风险,提高医嘱合格率,进一步保障用药安全。
Objective:In order to improve the level of clinical rational drug use,improve the efficiency and quality of pharmacist prescription review,build the platform of hospital prescription review center,and discuss its application effect in clinical rational drug use.Methods:Introduce the prescription review center system to assist pharmacists in prescription/order review by establishing and maintaining the knowledge base of the system.Results:From June 2018 to August 2019,1833 medical orders were directly intercepted by the system level 8,368,880 medical orders were manually reviewed by pharmacists,and 1,383 medical orders were intervention orders.Conclusion:By using the platform of prescription review center,the pre-approval of medical orders can effectively and timely intercept unreasonable prescriptions,reduce the risk of clinical medication errors,improve the qualified rate of medical orders,and further guarantee the medication safety of patients.
