摘要
目的分析反复喘息患儿血清中25羟维生素D3水平与γ干扰素(IFN-γ)、白介素4(IL-4)、白介素10(IL-10)及免疫球蛋白E(IgE)水平之间的相关性,探讨25羟维生素D3参与反复喘息的发病机制。方法选取2018年3月至2019年3月于我院就诊的6月至5岁龄儿童,分为反复喘息组(包括API指数阳性组和API指数阴性组)、肺炎组及健康组,采用质谱法检测患儿血清中25羟维生素D3浓度,流式细胞学方法检测血清IFN-γ/IL-4及IL-10水平,ELISA方法检测血清IgE水平。结果反复喘息组25羟维生素D3水平低于肺炎组和健康组,且API指数阳性组更低(P<0.05),肺炎组与健康组差异无统计学意义(P>0.05);反复喘息组IFN-γ/IL-4及IL-10水平低于肺炎组和健康组,且API指数阳性组更低,肺炎组高于健康组(P<0.05);反复喘息组IgE水平高于肺炎组和健康组,且API指数阳性组更高,肺炎组高于健康组(P<0.05);反复喘息组血清25羟维生素D3水平与IFN-γ/IL-4、IgE水平呈明显负相关,与IL-10水平呈明显正相关(P<0.05)。结论儿童反复喘息的发病与血清中25羟维生素D3浓度低下有关,这可能是发生IFN-γ/IL-4、IL-10及IgE水平变化的主要原因,即25羟维生素D3浓度低下可能引发变态反应,同时25羟维生素D3水平与API预测指数在评估反复喘息患儿是否发展为哮喘上有一致性。
Objective To analyze the correlation between serum 25-hydroxyvitamin D3 and levels of IFN-γ/IL-4,IL-10 and IgE in children with recurrent wheezing,and to explore the pathogenesis of 25-hydroxyvitamin D3 in recurrent wheezing.Methods From March 2018 to March 2019,children aged 6 months to 5 years old in our hospital were selected and divided into repeated wheezing group(including API positive group and API negative group),pneumonia group and healthy group.Serum 25-hydroxyvitamin D3 levels were detected by using mass spectrometry,levels of IFN-γ,IL-4 and IL-10 were detected by using flow cytometry and IgE levels were detected by using ELISA.Results The level of 25-hydroxyvitamin D3 in the repeated wheezing group was lower than that in the pneumonia group and the healthy group,and API index positive group was much lower(P<0.05).There was no significant difference between the pneumonia group and the healthy group(P>0.05).Levels of IFN-γ/IL-4 and IL-10 in the repeated wheezing group was lower than those in the pneumonia group and the healthy group,and the API index positive group was much lower while the pneumonia group was higher than the healthy group(P<0.05).IgE level in the repeated wheezing group was higher than that in the pneumonia group and the healthy group,and the API index positive group was much higher while the pneumonia group was higher than the healthy group(P<0.05).Conclusion The occurrence of recurrent wheezing in children is related to the low level of 25-hydroxyvitamin D3,and the low level of serum 25-hydroxyvitamin D3 may be the main reason for the changes of IFN-γ/IL-4,IL-10 and IgE levels.In another words,low level of 25-hydroxyvitamin D3 may aggravate the occurrence of allergy.25-hydroxyvitamin D3 level is consistent with API prediction index in assessing whether children will develop recurrent wheezing.
