摘要目的:比较后循环短暂性脑缺血发作(TIA)与后循环急性脑梗死(ACI)患者椎动脉内径(VAD)、收缩期血流峰值(PSV)及颈动脉内-中膜厚度(CIMT)的差异。方法:回顾性分析2017年5月-2018年5月本院确诊的86例后循环TIA患者(后循环TIA组)与56例后循环ACI患者(后循环ACI组)的临床资料,均行颈动脉彩色多普勒超声检查。比较两组患者VAD、PSV及CIMT的差异。结果:后循环TIA组双侧VAD和PSV均明显低于后循环ACI组,差异均有统计学意义(P<0.01);后循环TIA组双侧颈内、外动脉PSV均明显低于后循环ACI组,差异均有统计学意义(P<0.01)。后循环TIA组双侧CIMT均明显低于后循环ACI组,差异均有统计学意义(P<0.01)。结论:后循环TIA与后循环ACI患者在双侧椎动脉和颈内、外动脉血流及动脉硬化程度方面存在一定差异,而颈动脉彩色多普勒超声检查对这两种缺血性脑血管病均具有良好的评估价值。 Objective:To compare the differences of vertebral artery diameter(VAD),peak systolic velocity(PSV)and carotid intima-media thickness(CIMT)between patients with posterior circulation transient ischemic attack(TIA)and acute cerebral infarction(ACI).Method:The clinical data of 86 patients diagnosed with posterior circulation TIA(posterior circulation TIA group)and 56 patients diagnosed with posterior circulation ACI(posterior circulation ACI group)in our hospital from May 2017 to May 2018 were retrospectively analyzed.All patients were examined by color Doppler ultrasound.The differences of VAD,PSV and CIMT between the two groups were compared.Result:Both VAD and PSV of TIA group were significantly lower than ACI group,the differences were statistically significant(P<0.01).PSV of bilateral internal and external carotid arteries in TIA group were significantly lower than those in ACI group,the differences were statistically significant(P<0.01).CIMT in both sides of TIA group were significantly lower than ACI group,the differences were statistically significant(P<0.01).Conclusion:There are some differences between patients with posterior circulation TIA and those with posterior circulation ACI in the blood flow of bilateral vertebral artery,internal and external carotid artery and the degree of arteriosclerosis,and color Doppler ultrasonography of carotid artery has a good evaluation value for both ischemic cerebrovascular diseases.
