摘要
目的 拓展心理健康服务范围,维护社区居民精神心理健康.方法 依托安徽省创新城市医联体平台,结合临床经验和社区调研结果,构建以护士为主导的医院—社区精神心理健康服务模式,选派具有心理咨询师或心理治疗师资质的高年资护士下沉社区,开设“阳光心理工作室”;横断面调查社区人群焦虑、抑郁、睡眠现状及需求,确立服务项目;对独居老人、失独家庭、留守儿童等特殊人群进行重点监管,实施心理状况动态评估和干预;设计特色“心理健康维护服务包”进行个性化签约服务;采用“标准化病人”联合微课形式,对试点社区医护人员进行精神心理专业培训.结果 运行1年半,“阳光心理工作室”接诊人数逐步增加,累计达703人次,112名社区居民签约心理健康维护服务包;社区医护人员精神心理疾病知识、心理健康评估技能、精神症状风险评估技能均较培训前显著提高(均P<0.01).结论 以护士为主导的医院—社区精神心理健康服务模式,能够填补社区心理健康服务供给不足的现状,提高社区居民心理健康素养.
Objective To extend the scope of mental health service,and to maintain mental health of community-dwelling residents.Methods Based on the innovative urban medical alliance platform in Anhui province,by combining with clinical experience and results of pilot survey in the communities,we constructed a nurse-led hospital-community mental health service model.Senior nurses with psychological consultant or psychotherapist certification were designated to the community to run a clinic of psychological consulting.Tasks of the senior nurses included the following aspects:conducting cross-sectional survey to explore the levels of anxiety,depression and sleep quality among the community residents and their needs,then identifying service contents;providing focused monitoring for such vulnerable populations as solitary elderly people,the families losing the only child and the left-behind children,and implementing dynamic psychological evaluation and intervention for them;designing distinctive mental health service to provide individualized service;and giving specialty training for community healthcare personnel by employing standardized patient and micro-lectures.Results Community residents′attendance to the clinic gradually increased,totaling to 703 accounts within one and half year,and 112 residents signed for mental health service.The levels of knowledge of mental illness,mental health assessment skills,and mental symptom risk assessment skills of community healthcare personnel were significantly improved(P<0.01 for all).Conclusion The nurse-led hospital-community mental health service model can supplement community mental health service,thus to enhance mental health of community-dwelling residents.
