摘要
目的探讨自体脂肪填充术治疗轻度阴道松弛症的可行性及临床疗效。方法自2013年6月至2018年4月选择门诊就诊的18例无分娩史、非阴道分娩、无产伤及外伤史的轻度阴道松弛症患者行阴道自体脂肪填充术。获取患者自身脂肪组织约15 ml,将纯化的脂肪组织平铺注射于会阴体及阴道外口;术后给予抗炎治疗,禁止性生活2个月。结果术后3个月,18例患者均无感染、液化、囊肿、硬结形成等并发症,阴道外口形态饱满,阴道内可容纳2指至2指半,其中15例(83.3%)患者自觉性生活改善;2例(11.1%)患者自觉改善不明显;1例(5.6%)患者术后无性生活。17例患者手术前后的FSFI评分结果显示,从术前20.02分提高到术后26.05分(P<0.01)。盆腔MRI检查示:术后3、6个月阴道后壁近外口处注射的脂肪组织存活良好,体积维持较好。结论阴道自体脂肪填充术是一项安全、可行的手术方法,操作简便,损伤较小,术后恢复较快,是矫正轻度阴道松弛症的有效方法之一。
Objective To explore the feasibility and clinical efficacy of autologous fat grafting in the treatment of mild vaginal relaxation. Methods From June 2013 to April 2018, 18 outpatients with mild vaginal relaxation(no delivery history, vaginal delivery, birth injuries and trauma) underwent autologous fat grafting. About 15 ml of autologous fat tissue were harvested from each patient, and the purified fat was injected into perineal body and vaginal external orifice. Anti-inflammatory agent was given after operation and sexual life was prohibited for 2 months. Results There were no obvious complications such as infection, liquefaction, cyst and induration occurred at 3 months after operation. The appearance of vaginal external orifice was full and 2.0 to 2.5 fingers could be accommodated in the vagina.Fifteen patients(83.3%) thought their sexual life was improved, 2 patients(11.1%) thought the improvement of their sexual life was not obvious, and 1 patient(5.6%) had no sexual life after operation. The FSFI scores of 17 patients increased from 20.02 pre-operative to 26.05 post-operative(P<0.01). The MRI examination of pelvic cavity showed that the injected fat tissue at posterior vaginal wall near the external orifice survived well at 3 and 6 months after operation. The volume was maintained well. Conclusion Autologous fat grafting was a safe and feasible method for mild vaginal relaxation. This operation was simple to perform, the injury was small and the recovery was quick. Autologous fat grafting was one of the effective methods to treat mild vaginal relaxation.
