摘要
目的探讨不切除黏膜阴道后壁皱褶缝合阴道紧缩术治疗中重度阴道松弛的临床疗效。方法选取2021年10月至2023年1月在南阳市某三甲医院接受阴道紧缩术的83例中重度阴道松弛患者为研究对象,根据术中是否切除黏膜,将其分为观察组45例和对照组38例,其中观察组实施不切除黏膜阴道后壁皱褶缝合阴道紧缩术,对照组实施部分阴道黏膜切除。比较两组手术相关指标、临床疗效和性功能改善情况。结果所有患者均顺利完成手术,观察组术中出血量较对照组少,组间比较差异具有统计学意义(t=5.209,P<0.01);两组手术时间、术后疼痛评分、住院总时间比较,差异均无统计学意义(P>0.05)。观察组术后治愈20例,改善20例,无效5例,治疗有效率88.89%,对照组术后治愈18例,改善16例,无效4例,治疗有效率89.47%,差异无统计学意义(χ^(2)=0.072,P=0.788)。术后3个月,观察组性唤醒、阴道润滑度、性高潮、性满意度、性交痛和性生活质量评分均高于对照组,差异具有统计学意义(P<0.05)。结论阴道黏膜切除与保留均能够有效改善阴道松弛程度,均是临床阴道松弛的有效术式,不切除阴道黏膜阴道后壁皱褶缝合术能够减少术中出血量,最大限度保留阴道性刺激敏感性,对于提升性生活质量具有良好效果。
Objective Exploring the clinical efficacy of vaginal tightening surgery without removing mucosal folds on the posterior wall of the vagina for the treatment of moderate to severe vaginal relaxation.Methods 83 patients with moderate to severe vaginal relaxation who underwent vaginal tightening surgery at a tertiary hospital in Nanyang City from October 2021 to January 2023 were selected as the research subjects.They were divided into an observation group of 45 cases and a control group of 38 cases based on whether the mucosa was removed during the surgery.The observation group underwent vaginal tightening surgery without removing the mucosa,and the control group underwent partial vaginal mucosal resection.Compare the surgical related indicators,clinical efficacy,and sexual function improvement between two groups.Results All patients successfully completed the surgery,and the observation group had less intraoperative bleeding compared to the control group.The difference between the groups was statistically signifificant(t=5.209,P<0.01);There was no statistically signifificant difffference in surgical time,postoperative pain score,and total hospital stay between the two groups(P>0.05).The observation group received postoperative cure in 20 cases,improvement in 20 cases,and ineffffective in 5 cases,with a treatment effffectiveness rate of 88.89%.The control group received postoperative cure in 18 cases,improvement in 16 cases,and ineffffective in 4 cases,with a treatment effffectiveness rate of 89.47%.The difffference was not statistically signifificant(χ^(2)=0.072,P=0.788).Three months after operation,the scores of sexual arousal,vaginal lubrication,Orgasm,sexual satisfaction,sexual intercourse pain and sexual life quality in the observation group were significantly higher than those in the control group(P<0.05).Conclusion oth vaginal mucosal resection and preservation can effffectively improve the degree of vaginal relaxation,and are effffective surgical methods for clinical vaginal relaxation.Suturing the posteri
