心脏彩色超声联合颈动脉超声检查在疑似冠心病诊断中的意义 被引量:25

The diagnostic value of color doppler echocardiography combined with carotid artery ultrasound in coronary heart disease

摘要 目的探讨心脏彩色超声联合颈动脉超声在冠心病诊断中的价值。方法选取2015年1月至2018年10月在隆昌县人民医院就诊的疑似冠心病167例,最后经冠状动脉造影确诊冠心病患者108例,非冠心病患者59例,均给予心脏彩色超声、颈动脉超声检查。结果冠心病组Tei指数、颈总动脉内膜厚度(intima thickness,IMT)和斑块积分分别为(0.57±0.04)、(1.32±0.21)mm和(2.27±0.87)分,明显高于非冠心病组(t=16.73、19.45、11.68;P<0.05);冠心病三支病变患者Tei指数、IMT及斑块积分分别为(0.62±0.06)、(1.44±0.23)mm和(2.81±0.92)分,明显高于单支病变和双支病变组(F=13.28、20.01、23.20;P<0.05);双支病变患者Tei指数、IMT及斑块积分分别为(0.56±0.05)、(1.30±0.19)mm和(2.18±0.87)分,明显高于单支病变(P<0.05);Tei指数、IMT及斑块积分诊断冠心病的ROC曲线下面积为(0.75、0.81和0.78,P<0.05);联合诊断冠心病的灵敏度、特异度、阳性预测值和阴性预测值分别为94.44%、42.37%、75.00%和80.65%。结论心脏彩色超声联合颈动脉超声在协助冠心病诊断中有较高的应用价值。 Objective To explore the value of color doppler echocardiography combined with carotid artery ultrasound in the diagnosis of coronary heart disease.Methods 167 patients with suspected coronary heart disease in Longchang People’s Hospital from January 2015 to October 2018 were selected.By coronary angiography,108 patients were diagnosed as coronary heart disease and 59 patients were diagnosed as non-coronary heart disease.All patients were examined by color doppler echocardiography and carotid artery ultrasound.Results The Tei index,carotid artery intima thickness(IMT)and plaque score in CHD group were(0.57±0.04),(1.32±0.21)mm and(2.27±0.87),which were significantly higher than those in non-CHD group(t=16.73,19.45,11.68;P<0.05).The Tei index,IMT and plaque score of patients with three vessel lesions of coronary heart disease were(0.62±0.06),(1.44±0.23)mm and(2.81±0.92),which were significantly higher than those of patients with single vessel lesion and double vessel lesion(F=13.28,20.01,23.20;P<0.05).The Tei index,IMT and plaque score of patients with double vessel disease were(0.56±0.05),(1.30±0.19)mm and(2.18±0.87),which were significantly higher than those of patients with single vessel disease(P<0.05).The area under ROC curve of Tei index,IMT and plaque integral in diagnosis of coronary heart disease was 0.75,0.81 and 0.78(P<0.05).The sensitivity,specificity,positive predictive value and negative predictive value of combined diagnosis of coronary heart disease were 94.44%,42.37%,75.00%and 80.65%.Conclusion Color doppler echocardiography combined with carotid artery ultrasound has a high application value in the diagnosis of coronary heart disease.
