神经梅毒患者血清和脑脊液中的CXCL13和TH1TH2细胞因子的临床意义

摘要 目的探讨神经梅毒患者血清和脑脊液中的CSF-B细胞趋化因子配体13(CXCL13)和TH1、TH2细胞因子的临床意义。方法选取2018年3~9月收治的30例无症状神经梅毒患者、30例轻瘫性神经梅毒患者为研究对象,并选取同期入院体检的30例健康人群为健康对照组,无症状神经梅毒组、轻瘫性神经梅毒组患者取血液、脑脊液样本,健康对照组仅取血液样本,采用ELISA法测定血液、脑脊液样本中细胞因子CXCL13、TH1、TH2水平并进行比较。结果无症状神经梅毒组、轻瘫性神经梅毒组血清CXCL13水平高于健康对照组、TH1水平低于健康对照组、TH2水平高于健康对照组,差异均有统计学意义(P<0.05);无症状神经梅毒组、轻瘫性神经梅毒组血清CXCL13、TH1、TH2水平差异无统计学意义(P>0.05)。轻瘫性神经梅毒组受试者脑脊液中CXCL13水平高于无症状神经梅毒组(P<0.05)。轻瘫性神经梅毒组脑脊液中TH1水平低于无症状神经梅毒组、TH2水平高于无症状神经梅毒组,差异均有统计学意义(P<0.05)。结论神经梅毒患者血清及脑脊液中CXCL13、TH2水平升高,TH1水平降低,且相较于无症状神经梅毒患者,轻瘫性神经梅毒患者脑脊液中这一趋势更为明显。 Objective To investigate the clinical significance of CSF B cell chemoattractant chemokine ligand 13(CXCL13)and TH1,TH2 cytokines in serum and cerebrospinal fluid of patients with neurosyphilis.Methods Thirty patients with asymptomatic neurosyphilis and 30 patients with paretic neurosyphilis,who were treated in out hospital from March to September 2018,were enrolled in research group and 30 healthy cases who underwent healthy examination during the same period were enrolled as control group.The blood and cerebrospinal fluid samples of the patients in research group and the blood samples of the patients in control group were taken,respectively.The levels of cytokines CXCL13,TH1 and TH2 in the blood and cerebrospinal fluid samples were determined by ELISA and compared among the three groups.Results The serum levels of CXCL13 and TH2 in research group were higher than those in control group,whereas the levels of TH1 in research group were significantly lower than those in control group(P<0.05).Moreover,there were no significant differences in the serum levels of CXCL13,TH1 and TH2 between the two case groups(P>0.05).The levels of CXCL13 and TH2 in cerebrospinal fluid of the patients in paretic neurosyphilis group were significantly higher than those in asymptomatic neurosyphilis group(P<0.05).However,the levels of TH1 in cerebrospinal fluid in paretic neurosyphilis group were significantly lower than those in asymptomatic neurosyphilis group(P<0.05).Conclusion The levels of CXCL13 and TH2 in serum and cerebrospinal fluid of patients with neurosyphilis are increased,and TH1 levels are decreased,which is more obvious in patients with paretic neurosyphilis,as compared with the patients with asymptomatic neurosyphilis.
