摘要
目的探讨肺结核合并糖代谢紊乱患者的生活质量并分析其影响因素。方法选取2018年5月-2019年2月新疆某二级甲等医院肺病科接受抗结核治疗的肺结核合并糖代谢紊乱患者,采用一般资料调查表、健康调查简表(SF-36)和Herth希望量表调查患者生活质量及希望水平,采用SPSS 24.0分析数据。结果124例调查对象中生活质量一般健康状况、生理机能、生理职能、躯体疼痛、精力、社会功能、情感职能和精神健康得分分别为43.78±12.03、56.61±19.93、51.41±17.84、51.72±10.95、59.48±12.92、57.22±20.95、45.16±24.49、51.56±11.76,其中一般健康状况和情感职能受损较为严重。单因素分析显示,不同性别、年龄分组、户籍所在地、月收入、学历及有无吸烟饮酒史的患者生活质量得分差异均有统计学意义(均P<0.05)。多元线性回归分析结果显示,性别、饮酒史、月收入及年龄为患者生活质量的独立影响因素(均P<0.05)。希望水平总得分为35.20±3.32,处于中低水平。结论肺结核合并糖代谢紊乱患者生活质量及希望水平低下,且受到多种因素共同影响。因此,在临床工作中应注意重点观察老年患者及不良生活方式的患者,加强对患者的健康宣教、心理护理,以帮助患者树立战胜疾病信心和改变不良生活方式,提高患者的生活质量。
Objective To discuss the quality of life in patients with pulmonary tuberculosis complicated with glucose metabolism disorder,analyze its influencing factors.Methods The patients with pulmonary tuberculosis complicated with glucose metabolism disorder,who received anti-tuberculosis treatment in lung department of a grade-A secondary hospital in Xinjiang from May 2018 to February 2019,were investigated by the general data questionnaire,the health survey summary form(SF-36)and Herth hope scale to obtain patients'quality of life and hope level.The data were analyzed by SPSS 24.0.Results Among the 124 subjects,the score of the general health status,physiology,physiological function,physical pain,energy,social function,emotional function and mental health in the life quality was 43.78±12.03,56.61±19.93,51.41±17.84,51.72±10.95,59.48±12.92,57.22±20.95,45.16±24.49 and 51.56±11.76,respectively,in which the general health and emotional functions were more severely damaged.The single factor analysis showed there were statistically significant differences in quality of life scores among patients with different gender,age,household registration,family monthly income,education and smoking and drinking history(all P<0.05).The multiple linear regression analysis showed gender,history of drinking,monthly income and age were the influencing factors(all P<0.05).The total score of hope level was 35.20±3.32,which was at the middle low level.Conclusion Patients with pulmonary tuberculosis and glucose metabolism disorders have low quality of life and low hope levels,and are affected by many factors.Therefore,in clinical work,it is necessary to pay attention to the observation of elderly patients and patients with unhealthy lifestyles,and strengthen the health education and psychological care to patients to help them establish confidence in disease and change bad lifestyles,so as to improve patients'quality of life.
