摘要
目的探讨脑脊液中β2-微球蛋白、超氧化物歧化酶(SOD)、铁蛋白检测在中枢神经系统白血病(CNSL)诊断中的应用价值。方法将2014年11月至2018年11月南京医科大学附属逸夫医院确诊的138例白血病患者纳入研究,其中CNSL患者70例(CNSL组),非CNSL患者68例(非CNSL组)。将同期该院收治的65例血液系统正常的脑外科患者作为对照组。分析比较上述3组人群脑脊液中β2-微球蛋白、SOD及铁蛋白水平,并分析单独或联合检测脑脊液中β2-微球蛋白、SOD、铁蛋白对CNSL的诊断效能。结果与对照组比较,白血病患者脑脊液中β2-微球蛋白、SOD、铁蛋白水平均显著升高(P<0.05),且CNSL组患者脑脊液中β2-微球蛋白、SOD、铁蛋白水平明显高于非CNSL组患者(P<0.05)。铁蛋白、β2-微球蛋白、SOD 3项指标联合检测灵敏度、特异度、准确度最高,此时ROC曲线下面积最大。结论联合检测脑脊液中β2-微球蛋白、SOD、铁蛋白水平能明显提高CNSL的诊断准确度、灵敏度及特异度,该方法值得推荐。
Objective To study on the application value of combined detection ofβ2-microglobulin,superoxide dismutase(SOD)and ferritin in cerebrospinal fluid in the diagnostic process of central nervous system leukemia(CNSL).Methods From November 2014 to November 2018,138 patients with leukemia were diagnosed,who were enrolled as study object in Sir Run Run Hospital,Nanjing Medical University.There were 70 cases of CNSL(CNSL group)and 68 cases of non-CNSL(non-CNSL group).At the same time,65 brain surgical patients with healthy blood system in the hospital were enrolled as the control group(n=65).Theβ2-microglobulin,SOD and ferritin in cerebrospinal fluid in the above three groups were compared and analyzed.At the same time,the diagnostic efficacy ofβ2-microglobulin,SOD and ferritin in cerebrospinal fluid alone or in combination detection was analyzed statistically.Results Compared with the control group,the levels ofβ2-microglobulin,SOD and ferritin in cerebrospinal fluid in patients with leukemia were significantly higher than those in the control group(P<0.05).The levels ofβ2-microglobulin,SOD and ferritin in cerebrospinal fluid in CNSL group were significantly higher than those in non-CNSL group.The sensitivity,specificity and accuracy were highest by combining detection ofβ2-microglobulin,SOD and ferritin in cerebrospinal fluid compared with single detection.What is more,the area under the ROC curve was the largest when combining detection ofβ2-microglobulin,SOD and ferritin in cerebrospinal fluid.Conclusion It can improve the diagnostic accurately,sepcificity and sensitively by of CNSL combined detection ofβ2-microglobulin,SOD and ferritin in cerebrospinal fluid,and this method is worthy of recommendation.
