摘要
目的:探讨磁共振高分辨技术(HRMRI)在脑梗死复发患者颈动脉斑块评估中的应用效果。方法:选取河南省职工医院2016年7月至2018年6月期间收治的118例脑梗死患者.将其中72例脑梗死复发患者作为观察组,将46例初发性脑梗死患者作为对照组。所有患者均行HRMRI检査,对比两组斑块稳定性、颈动脉斑块负荷参数、脂质坏死核心、斑块内出血、纤维帽破裂及管腔狭窄发生率。结果:观察组不稳定性斑块检出率较对照组高,差异有统计学意义(P<0.05);两组血管总面积(TVA)、管腔面积(LA)水平对比,差异无统计学意义(P>0.05);观察组管壁标准化指数(NWI)水平较对照组高,差异有统计学意义(P<0.05);观察组斑块内岀血、纤维帽破裂发生率较对照组高,差异有统计学意义(P<0.05);两组脂质坏死核心、管腔狭窄发生率对比,差异无统计学意义(P>0.05)。结论:脑梗死初发与复发患者的颈动脉斑块存在差异,采用HRMRI监测具有一定的诊断价值,有助于评估脑梗死复发风险。
Objective:To investigate the effect of high-resolution magnetic resonance imaging(HRMRI)in the assessment of carotid plaque in patients with recurrent cerebral infarction(RCI).Methods:118 patients with cerebral infarction in our hospital from July 2016 to June 2018 were selected.72 patients with RCI as the observation group and 46 patients with primary cerebral infarction as the control group.HRMRI was performed in all patients,and the plaque stability,plaque load parameters,lipid necrosis core,bleeding in plaque,fiber cap rupture and lumen stenosis were compared between the two groups.Results:The detection rate of unstable plaque in the observation group was higher than that in the control group,and the difference was statistically significant(PV0.05);The TVA and La levels in the two groups were not statistically significant(P>0.05);the NWI level in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05);The incidence of bleeding and fiber cap rupture in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference between the two groups(P>0.05).Conclusion:There is difference in carotid plaques between patients with primary cerebral infarction and RCI.HRMRI monitoring has certain diagnostic value and is helpful to assess the risk of recurrence of cerebral infarction.
