摘要
目的探讨社区脑卒中吞咽障碍患者生存质量与照护需求现状及其影响因素。方法采取方便抽样法,选择2017年8月至2018年8月广东省广州市各社区脑卒中吞咽障碍患者,采用吞咽障碍筛查工具(Eat-10)对其进行吞咽功能筛查,评定为吞咽障碍,最终纳入200例患者进入调查。调查影响患者生存质量的因素,以及照护需求现状的情况并进行分析,了解不同程度吞咽障碍患者的不同进食方式对患者生存质量的影响。结果不同性别、年龄、婚姻状况、职业、居住地、家庭人均月收入、脑卒中分期、吞咽治疗情况、吞咽障碍程度、进食方式患者生存质量各维度得分差异有统计学意义(均P<0.05)。多元回归分析结果:脑卒中分期、吞咽障碍程度、年龄、进食方式等是影响患者生存质量的主要因素;婚姻状况、吞咽障碍程度、年龄、文化程度、职业等是影响患者护理需求的主要因素。结论缺血性脑卒中、>69岁、重度吞咽障碍、鼻饲的患者生存质量较低。离婚或丧偶、>69岁、重度吞咽障碍、初中及以下文化程度、职业为农民的患者护理需求较高。
Objective To investigate the status of life quality and care needs of patients with stroke and dysphagia in community and their influencing factors.Methods Convenient sampling method was used to select patients with dysphagia from various communities in Guangzhou,Guangdong Province from August 2017 to August 2018.The swallowing function was screened by 10-Item Eating Assessment Tool(EAT-10),and 200 patients were evaluated as swallowing disorder and finally included in the investigation.The factors affecting the quality of life and care needs of patients were investigated,and the impacts of different eating patterns of patients with different levels of dysphagia on the quality of life were analyzed.Results There were significant differences in the scores of patients with different genders,age,marital status,occupation,place of residence,family monthly income,stroke stage,swallowing treatment,dysphagia,and eating mode(all P<0.05).Multiple regression showed that:stroke staging,dysphagia,age,and eating patterns were the main factors affecting the quality of life of patients,and marital status,degree of dysphagia,age,education level,occupation.were the main factors affecting the patients’nursing needs.Conclusion The patients with ischemic stroke,over 69 years old,severe dysphagia and nasogastric feeding had lower quality of life.The patients who are divorced or widowed,over 69 years old,with severe dysphagia,with junior high school education or below,and whose occupation is farmers have higher nursing needs.
