摘要
目的:探讨缩短儿内科病人出院当日滞留在院时间的各项措施,完善出院流程,便于出院患儿快速便捷完成出院手续并离开医院,利于新入院患儿能及时得到入住。方法:运用PDCA对造成儿内科患儿出院日滞留院内的因素进行分析,寻找主要原因,制定相应的措施,多团队协作进行落实改进。结果:造成出院日患儿滞留在院的主要原因有预出院流程未细化、预出院制度未落实;医生未及时开具出院医嘱,重复开药及新开辅助检查;护理人员未及时发放药物及进行出院宣教;患儿家属未及时完成辅助检查及等人来接等。通过医疗、护理、工勤等多团队协作,实施细化后的出院流程,并进行全员培训,建立健全监督机制等,病人延迟出院率由95.1%下降至31.3%,滞留在院时间由105.7分钟减少至15分钟。结论:运用PDCA方法,在多团队协作下,职责落实到实处,可以明显缩短儿内科病人出院当日滞留在院时间,建立完善的出院流程,可以在儿内科推广此方法。
Objective: To explore various measures to shorten the length of stay of pediatric patients on the day of discharge, improve the discharge process, facilitate discharge of children discharged from the hospital quickly and easily, and facilitate the timely admission of newly admitted children. Methods: The PDCA was used to analyze the factors that cause children to stay in the hospital on the day of discharge, to find the main reasons, to formulate corresponding measures, and to implement cooperation and improvement in multiple teams. Results: The main reasons for the children staying in the hospital on the discharge day were that the pre-discharge process was not detailed and the pre-discharge system was not implemented;the doctor did not issue a discharge order in time, repeated prescribing and new auxiliary examinations;the nursing staff did not release the drugs and Discharged the mission;the children's family members did not complete the auxiliary examination in time and waited for someone to pick up. Through the cooperation of multiple teams such as medical care, nursing, and labor, implement the detailed discharge process, carry out training for all staff, establish a sound supervision mechanism, etc., the delayed discharge rate of patients decreased from 95.1% to 31.3%, and the length of stay in hospital was 105.7 The minutes are reduced to 15 minutes. Conclusion: Using the PDCA method, with multi-team collaboration, fulfilling responsibilities, can significantly reduce the length of stay of pediatric patients on the day of discharge, establish a complete discharge process, and promote this method in pediatrics.
