摘要
目的探讨纳布啡对降低全身麻醉诱导期间舒芬太尼引发咳嗽的临床效果。方法选择2018年1-6月天津市滨海新区中医医院和天津市北辰医院80例拟行气管插管的全身麻醉患者,美国麻醉医师协会(ASA)分级Ⅰ~Ⅱ级,将其分为观察组和对照组,每组40例。观察组预先静脉注射纳布啡0.1 mg/kg(生理盐水稀释至10 mL),对照组预注等容积生理盐水,注射时间5 s。5 min后两组患者均静脉注射舒芬太尼0.5μg/kg,2 min后给予其他诱导药物,肌肉松弛满意后行气管插管。观察两组患者注射纳布啡或生理盐水前(T 1)、注射舒芬太尼前(T 2)、插管前(T 3)的平均动脉压(MAP)和心率(HR)。比较两组患者全身麻醉诱导期间咳嗽的发生例数、次数和程度。结果两组患者T 1与T 2时间点MAP、HR水平比较,差异无统计学意义(P>0.05);两组患者T 3时间点MAP、HR水平均低于T 1、T 2时间点,组内比较差异有统计学意义(P<0.05)。与对照组比较,观察组患者在注射舒芬太尼2 min内咳嗽发生率降低、咳嗽程度减轻,差异均有统计学意义(P<0.05)。结论预注0.1 mg/kg纳布啡能有效预防舒芬太尼全身麻醉诱导引起的咳嗽,且对血流动力学无明显影响。
Objective To investigate the clinical effect of nabulphine for reducing sufentanil induced cough during the induction period of general anesthesia.Methods Eighty cases undergoing tracheal intubation for general anesthesia,ASA gradeⅠ-Ⅱ,in Binhai New Area Chinese Hospital and Tianjin Municipal Beichen Hospital from January to June 2018 were selected and divided into the observation group and control group,40 cases in each group.The observation group was pre-injected by nabulphine 0.1 mg/kg(normal saline was diluted to 10 mL),while the control group was pre-injected by the same volume of normal saline,the injection time was 5 s,after 5 min,sufentanyl 0.5μg/kg was intravenously injected,after 2 min the other inductive drugs were given,after muscle relaxation reached satisfaction,the tracheal intubation was performed.The mean arterial pressure(MAP)and heart rate before injetion of nabulphine or normal saline(T 1),before injection of sufentanil(T 2)and before tracheal intubation(T 3)were observed.The cases number of cough occurrence,frequency and degree were compared between the two groups.Results The MAP and HR levels in the two groups had no statistical difference between at T1 and T2(P>0.05);the MAP and HR levels at T3 in the two groups were lower than those at T 1 and T 2,the intra-group comparison had statistical significance(P<0.05).Compared with the control group,the cough occurrence rate within 2 min of sulfentanil injection in the observation group was decreased,cough degree was alleviated,and the differences were statistically significant(P<0.05).Conclusion Pre-injection of 0.1 mg/kg nabulphine can effectively prevent cough induced by sufentanil general anesthesia,moreover and has no obvious influence on hemodynamics.
