摘要
目的研究肾脑复元汤联合醒脑开窍针刺法治疗缺血性中风的临床效果。方法纳入2015年1月—2019年1月收治的124例缺血性中风患者为研究对象,按抽签随机方法分为两组,各62例,其中对照组予以规范治疗并配合醒脑开窍针刺疗法,观察组在对照组基础上予以肾脑复元汤治疗。比较两组中医证候疗效,采用美国国立卫生研究院卒中量表(NIHSS)及日常生活活动能力(ADL)量表,评估两组治疗前后神经功能损伤程度及日常生活活动能力,并比较治疗前后血浆黏度、全血比黏度高切、全血比黏度低切、纤维蛋白原、C反应蛋白、同型半胱氨酸水平及安全性。结果观察组总有效率为95.16%,明显高于对照组的83.87%,差异有统计学意义(P<0.05)。观察组治疗4周时NIHSS评分显著低于治疗前及对照组治疗4周时(P<0.05),ADL评分显著高于治疗前及对照组治疗4周时(P<0.05)。观察组治疗4周时全血比黏度高切值、全血比黏度低切值、血浆黏度及纤维蛋白原、C反应蛋白、同型半胱氨酸均显著低于治疗前及对照组治疗4周时(P<0.05)。两组治疗期间均未出现严重不良反应。结论肾脑复元汤联合醒脑开窍针刺疗法治疗缺血性中风疗效确切,能明显改善患者神经功能及生活自理能力,调节血液黏度,增加血液流动性,减轻炎症反应,且安全性较高。
Objective To investigate the effect of Shennao Fuyuan Decoction combined with brain-enlivening and orifice-opening needling in the treatment of ischemic stroke.Methods 124 patients with ischemic stroke admitted to our hospital from January,2015 to January,2019 were randomly divided into the observation group and the control group,with 62 patients in each group.The control group was given routine treatment and brain-enlivening and orifice-opening needling.The observation group was given brain-enlivening and orifice-opening needling combined with Shennao Fuyuan Decoction.National Institute of Health Stroke Scale(NIHSS)and active ability of daily life(ADL)scale were applied to evaluate the nerve function deficit and ability of daily life;plasma viscosity,whole blood high shear viscosity,whole blood low shear viscosity,fibrinogen,C-reactive protein,homocysteine level and safety were evaluated.Results The total effective rate in the observation group(95.16%)was significantly higher than that in the control group(83.87%),with significance(P<0.05).At the end of the 4 th week,NIHSS score in the observation group significantly decreased,which was significantly lower than that in the control group(P<0.05).At the end of the 4 th week,ADL score in the observation group significantly increased,which was significantly higher than that in the control group(P<0.05).The whole blood specific viscosity,whole blood specific viscosity,plasma viscosity,fibrinogen,C-reactive protein and homocysteine in the observation group were significantly lower than those before treatment and those in the control group at the 4 th week of treatment(P<0.05).No severe adverse reaction was found during the treatment.Conclusion Shennao Fuyuan Decoction combined with brain-enlivening and orifice-opening needling is effective in treating ischemic stroke,which can significantly improve the neurological function and self-care ability of the patients,regulate blood viscosity,increase blood fluidity,reduce inflammation with high safety.
