FAP和MMP-9在人乙肝相关性肝纤维化分级及肝癌中的表达意义 被引量:3

摘要 目的 研究成纤维细胞激活蛋白(FAP)和基质金属蛋白酶-9(MMP-9)在人乙肝相关性肝纤维化和肝癌组织中的表达,探讨其可能的作用机制.方法 收集符合筛选标准的74例标本入组,包括42例慢性乙肝相关性肝纤维化患者的肝穿刺标本和32例乙肝相关性肝癌患者术后的标本.采用HE和Masson染色判定肝纤维化程度,免疫组化检测FAP和MMP-9蛋白在肝纤维化和肝癌中的表达情况.结果 FAP和MMP-9表达在肝纤维化不同分级中均有差异(P=0.000).随肝纤维化程度增加,表达增强,至S3期达到高峰,S4期表达明显减弱.肝癌组织中均可见FAP和MMP-9的表达,高表达率分别为50%(16/32),43.8%(14/32),周围肝硬化组织中FAP和MMP-9基本上弱表达或不表达.FAP的高表达与肝癌患者的术前血清AFP、肿瘤数目、分化程度有相关性(P<0.05).相关性分析显示在肝纤维化和肝癌组织中FAP和MMP-9的表达呈正相关(P<0.05).结论 FAP和MMP-9可能协同促进肝星状细胞的活化,参与降解细胞外基质,从而促进肝纤维化和肝癌的发生发展. Objective To investigate the expression of fibroblast activation protein(FAP)and matrix metalloproteinase-9(MMP-9)in human HBV related hepatic fibrosis and cancer tissues and to illuminate its possible mechanism.Methods A total of 74 chronic hepatitis B patients were collected into the study,including 42 who received Ever—punctured biopsy with chronic liver fibrosis and 32 who underwent hepatectomy due to hepatoceDular carcinoma.HE and Masson were used to define the pathological degree of liver fibrosis.The expressions of FAP and MMP—9 were detected by immunohistochemical technique in human hepatic fibrosis and cancer tissues.Results The expression of FAP and MMP-9 were gradually increased along with liver fibrosis and reached the peak at S3 stage.It was weakened markedly at S4 stage.FAP and MMP—9 were seen in the total of 32 liver cancers.The high positive expression rates of FAP and MMP-9 were 50%(16/32)and 43.8%(14/32)respectively,while there was weak positive staining or no expression in surrounding hepatic tissue.The high posirive expression of FAP was correlated with the preoperative level of sertim AFP,tumor number and differentiation degree(P<0.05).Correlation analysis showed there were positive correlations between FAP and MMP—9 expression in both hepatic fibrosis and cancer tissues(P<0.05).Conclusion FAP and MMP-9 may promote the activation of hepatic stellate cells,degrade the extracellular matrix and accelerate the development of hepatic fibrosis and cancer.
