摘要
目的评价Er∶YAG激光联合5氟尿嘧啶(5 fluorouracil,5FU)和卤米松治疗白癜风疗效和安全性。方法白癜风患者70例,随机分为对照组和激光组,每组患者35例。对照组采用局部外用0. 05%卤米松乳膏联合5%5FU霜每日1次,每隔1 d使用1次,共治疗2个月;激光组,先行Er∶YAG激光治疗后,即刻和术后2 d均局部外用5%5FU霜,之后隔天局部外用0. 05%卤米松乳膏和5%5FU霜,每日1次,持续至2个月;激光治疗光斑大小3 mm,能量密度2~5 J/cm2。治疗2和4个月观察两组患者的临床疗效,测量白斑的面积,色素评分,并记录不良反应发生情况。结果两组患者治疗后2和4个月激光组总有效率好于对照组(P<0. 05)。与对照组比较,激光组在治疗后短时间内白斑面积明显减小,色素评分增加,差异均具有显著性意义(P<0. 01)。两组均无严重不良反应。结论 Er∶YAG激光可以帮助减少长期局部外用药物治疗的不良反应,可在较短时间内取得疗效,改善患者的依从性。
Objective To evaluate the efficacy and safety of combining Er∶YAG laser plus 5 fluorouracil(5 FU)and halomethasone in the treatment of vitiligo.Methods Totally 70 eligible patients were divided randomly into two groups:the control group given topical 5 FU and halometasone and the laser group given Er∶YAG laser in addition. The area of patch and pigmentation score were measured respectively 2 and 4 months after the therapy.Results The therapeutic effect of the laser group was better than that of the control group(P<0. 05). The reduction in the patch area and increase in pigment score of the laser group were significantly different from those in the control group(P<0. 01). No serious adverse reaction was observed in either group.Conclusions Greater reduction in the patch area and increase in pigmentation score was observed in the laser intervention group especially in short periods after the therapy. Er∶YAG is useful to reduce complications of long-term topical treatments,achieve faster response,and improve patients’ adherence.
