摘要
目的分析癌结节的起源、数目及其与Ⅲ期结直肠癌患者临床病理特征及预后的关系。方法回顾性分析2011年1月1日至2013年12月31日于宁夏医科大学总医院行根治性手术治疗的271例Ⅲ期结直肠癌患者的临床病理资料,Logistic回归法、Kaplan-Meier法、Log rank检验及Cox回归分析探讨癌结节与Ⅲ期结直肠癌患者预后的相关性。结果271例Ⅲ期结直肠癌患者中,癌结节阳性者91例(33.6%),N分期、肿瘤位置及组织学类型是癌结节存在的危险因素(P<0.05);癌结节是影响Ⅲ期结直肠癌患者无病生存时间的独立危险因素(P<0.05),癌结节阳性的Ⅲ期结直肠癌患者中位无病生存期明显低于癌结节阴性患者(26.0%vs.90.0%,P<0.01),癌结节阳性患者的死亡风险是非癌结节患者的2.215倍(95%CI:1.560~3.146,P<0.05);癌结节数目是影响癌结节阳性的Ⅲ期结直肠癌患者无病生存时间的独立危险因素,在癌结节阳性的患者中,癌结节数目>2的Ⅲ期结直肠癌患者的死亡风险是≤2患者的2.051倍(95%CI:1.191~3.531,P<0.05)。结论癌结节的形成可能与淋巴结转移相关,癌结节是Ⅲ期结直肠癌患者独立的不良预后因素,癌结节数目超过2个的Ⅲ期结直肠癌患者预后不良。
Objective To analyze the origin and number of tumor deposit and their relationship with dinicopathological features and the prognosis of patients with stageⅢcolorectal cancer.Methods We retrospectively analyzed the clinical and pathological data of 271 patients with colorectal cancer who underwent radical surgery from January 1,2011 to December 31,2013 in General Hospital of Ningxia Medical University.The correlation between tumor deposit and prognosis in the patients with stageⅢcolorectal cancer was analyzed by Logistic regression,Kaplan-Meier method,Log rank and Cox regression analysis.Results Among the 271 patients with stageⅢcolorectal cancer,91(33.6%)were positive for tumor deposit.N stage,tumor location and histological type were risk factors for tumor deposit(P<0.05).Tumor deposit was an independent risk factor for the disease-free survival time of patients with stageⅢcolorectal cancer(P<0.05).The median disease-free survival of patients with stage III colorectal cancer with tumor deposit was significantly lower than that of patients with negative tumor deposit(26.0%vs.90.0%,P<0.01);the risk of death in tumor deposit-positive patients was 2.215 times that in tumor deposit-negative(95%CI:1.560-3.146,P<0.05).The number of tumor deposits was an independent risk factor for disease-free survival in patients with stageⅢcancer of colorectal cancer who were positive for tumor deposit.In stageⅢcolorectal cancer patients with tumor deposit,the risk of death with tumor deposit>2 was 2.051 times that with tumor deposit≤2(95%CI:1.191-3.531,P<0.05).Conclusion The formation of tumor deposit may be associated with lymph nodes.Tumor deposit is an independent prognostic factor for stageⅢcolorectal cancer.Patients with stageⅢcolorectal cancer with more than two tumor deposits have a poor prognosis.
