笑气-氧气吸入镇静法在口腔颌面部表浅脓肿切开引流术中的临床观察 被引量:2

摘要 目的观察笑气-氧气吸入镇静法在口腔颌面部表浅脓肿切开引流术的临床应用效果。方法40例需切开引流口腔颌面部表浅脓肿的患者按随机数字表法分为实验组和对照组,每组20例;实验组在笑气-氧气吸入镇静下结合局部麻醉行脓肿切开引流术,对照组仅仅在局部麻醉下行脓肿切开引流术,分别记录两种方法的切开引流手术所需的时间,收缩压(systolic blood pressure,SBP)、舒张压(diastolic blood pressure,DBP)、心率(heart rate,HR)、呼吸频率(respiratory rate,RR)和指脉氧饱和度(pulse oxygen saturation,SpO2)变化,并进行疼痛程度(VAS评分)和镇静程度(Ramsay评分)的观察。结果实验组脓肿切开引流时间缩短,SBP、DBP、HR、RR、SpO2变化较对照组平稳,紧张焦虑、疼痛程度得到一定程度减轻,局麻和手术时VAS分别为(1.4±0.8)、(2.3±0.9);镇静程度增强,术前和术中Ramsay分别为(3.4±0.9)、(2.3±0.7),与对照组比较均有显著统计学差异(P<0.05)。结论笑气-氧气吸入镇静法具有减轻焦虑、减轻疼痛、安全、有效等特点,在口腔颌面部表浅脓肿切开引流术中,增加患者的舒适度具有一定的应用价值。 Objective To observe the clinical effects of nitrous oxide inhalation conscious sedation Abscess incision and drainage on superficial abscess in oral and maxillofacial region.Methods Forty patients,whoes superficial abscess in oral and maxillofacial region needed Incision and drainage,were randomly divided in to an experimental group(n=20)and a control group(n=20).The patients in the experimental group were subjected to Incision and drainage of superficial abscess in oral and maxillofacial region under nitrous oxide inhalation conscious sedation And local anesthesia,while the patients in the control group were subjected to Incision and drainage of Incision and drainage of superficial abscess in oral and maxillofacial region under routine local anesthesia.The duration of abscess Incision and drainage and the changes of systolic blood pressure(SBP),diastolic blood pressure(DBP),heart rate(HR),respiratory rate(RR)and pulse oxygen saturation(pulse oxygen saturation,SpO2)were recorded.The pain levels and sedation effects were analyzed with visual analogue scale(VAS)and Ramsay scale.Results The duration of abscess Incision and drainage in the experimental group was significantly shorter than that of the control group(10.5±3.0 min vs 17.1±4.5 min,P<0.05).SBP,DBP,HR,RR,and SpO2 changed more smoothly in The experimental group than in the control group.The local anesthesia and intraoperative VAS scores in the experimental group were significantly lower than those in the control group(1.4±0.8 and 2.3±0.9 vs 2.9±1.25.9±1.7,P<0.05),indicating reduced pain levels.The preoperative and intraoperative Ramsays cores in the experimental group were significantly higher than those in the control group(3.4±0.9 and 2.3±0.7vs 1.3±0.5 and 1.1±0.3,P<0.05),indicating increased sedation effects.Conclusion Nitrous oxide inhalation conscious sedation is safe and effective in Incision and drainage of superficial abscess in oral and maxillofacial region.
