摘要
目的应用眼前节OCT观察年龄相关性白内障患者行白内障超声乳化吸除联合人工晶状体植入术时,同期植入囊袋张力环对术后前房深度、房角、人工晶状体与后囊膜距离以及后囊膜混浊情况的影响。方法对安徽医科大学第二附属医院2017年5-10月34例(68只眼)双眼年龄相关性白内障患者,随机选择--眼作为植人组,在行白内障超声乳化吸除联合后房型人工晶状体植入术的同期植入囊袋张力环;对侧眼作为对照组,单纯行白内障超声乳化吸除联合人工晶状体植入术,术后随访6个月。结果34例患者术前及术后第2d、3个月、6个月两组LogMAR视力比较差异无统计学意义(P>0.05);术后2d两组前房深度、房角大小(TIA500μm)无差异(P>0.05),术后3、6个月前房深度、房角大小(TIA500μm)有差异(P<0.05),对照组ACD为(2.45+0.33).(3.77+0.36).(3.64+0.28).(3.63+0.29)mm,植人组ACD为(2.45+0.34)、(3.84+0.32)、(3.85+0.35)、(3.82+0.33)mm;对照组房角为(25.83土7.74)°、(43.35+6.55)0、(41.87+4.67)°、(42.49+4.29),植入组房角为(27.52+7.43)0、(45.40+5.23)°、(46.50+3.66)°、(45.40+3.70);术后2d,两组术后晶状体与后囊之间距离有差异(P<0.05),植人组明显小于对照组,术后2d、3个月两组均未发现人工晶状体偏位、倾斜,前囊膜未发现皱缩迹象,均未发现明显的后囊膜混浊。术后6个月植入组1只眼出现后囊膜混浊;对照组6只眼出现后囊膜混浊。结论对于年龄相关性白内障的患者,在行白内障超声乳化联合人工晶状体植入术的同期植人囊袋张力环,抑制囊袋皱缩,可以稳定人工晶状体,避免术后“近视漂移”,缩小术后人工晶状体与后囊之间距离,--定程度阻止晶状体上皮细胞向后囊膜增殖,从而减少后发性白内障的发生率。
Objective The application of anterior segment OCT was observed in patients with age-related cataract underwent phacoemulsification and intraocular lens implantation,implantation of capsular tension ring effeet on postoperative anterior chamber depth,anterior chamber angle,The distance between the intraocular lens and posterior capsule,posterior capsule opacification.Methods 34 patients in The Second Affiliated Hospital of Anhui Medical University from May 2017 to October 2017(68 eyes)with age-related cataract eyes,one eye randomly chosen one as implantation group,in phacoemulsification combined with intraocular lens implantation in the same period of capsular tension ring implantation;the contralateral eye as control group,treated with phacoemulsification combined with IOL implantation,the postoperative follow--up of 6 months.Results There was no significant difference in visual acuity between 34 patients before and second days,3 months and 6 months after operation(P>0.05),while in the control group,the LogMAR visual acuity was0.73+0.370.42+0.400.21+0.110.21+0.12,implantation group 0.71+0.390.37+0.240.22+0.100.22+0.10;2 day of the two groups of postoperative anterior chamber depth,the real angle(TIA500um)had no significeant difference(P>0.05),after 3 months and 6 months after surgery,anterior chamber depth,anterior chamber angle there are differences in size(TIA500um)(P<0.05),control group ACD 2.45+0.333.77+0.363.64+0.283.63+0.29,ACD implantation group2.45+0.343.84+0.323.85+0.353.82+0.33;the control group angle 25.83+7.7443.35+6.5541.87+4.6742.49+4.29,implantation group angle 27.52+7.4345.40+5.2346.50+3.6645.40+3.70;2 days after operation,there are differences between the two groups after the distance between the intraocular lens and posterior capsule(P<0.05),implantation group was significantly less than the control group;after 2 days,3 months of the two groups were not found in intraocular lens deviation,tilt,anterior capsule shrinkage found no signs,no obvious posterior capsule opacification;6 months after imp
