超声乳化白内障摘除术后凋亡抑制因子对角膜厚度和角膜内皮细胞密度的影响 被引量:4

Effect of apoptotic inhibitors on corneal thickness and corneal endothelial cell density after phacoemulsification

摘要 目的:探讨超声乳化白内障摘除术后角膜厚度和角膜内皮细胞密度的变化。方法:随机选取老年性白内障患者60例,按不同的年龄段分为3组,选取糖尿病性白内障及高度近视并发性白内障患者各30例,所有患者均进行白内障超声乳化摘除手术,手术前和手术中对三组不同类型的白内障患者进行一般检查;采用超声测厚仪对老年性白内障患者超声乳化白内障摘除术前和术后不同时间段的角膜厚度进行测量;采用非接触型角膜内皮显微镜对老年性白内障患者超声乳化白内障摘除术前和术后不同年龄、不同时间段、不同部位的角膜内皮细胞密度和不同类型白内障患者乳化白内障摘除术前和术后1个月检查角膜内皮细胞密度以及内皮细胞变异系数。结果:老年性白内障患者术后2小时、4天和15天术眼中央区角膜厚度较术前增加;老年性白内障患者术后4天时角膜内皮细胞密度较术前下降,术后15天~30天角膜内皮细胞密度无明显变化;老年性白内障患者术后角膜下部的内皮细胞密度减少的最多,与角膜上部、中部相比;70~79岁老年性白内障患者术后角膜内皮细胞密度下降平均值最大,与50~69岁患者相比;高度近视并发性白内障组及糖尿病性白内障组较老年性白内障组术后角膜内皮细胞密度降低、内皮细胞变异系数升高。结论:角膜细胞的凋亡能够反馈性的产生凋亡抑制因子,术后角膜厚度和内皮细胞密度维持在稳定的水平。 Objective To investigate the changes of corneal thickness and corneal endothelial cell density after phacoemulsification.Methods Sixty cases of senile cataract were randomly selected and divided into three groups according to different age groups.Thirty cases of diabetic cataract and thirty cases of high myopia complicated cataract were selected.All patients underwent phacoemulsification.Three different types of cataract patients were examined before and during the operation.Corneal thickness was measured before and after phacoemulsification in senile cataract patients of different ages by ultrasonic thickness gauge;different time periods and different parts of the cornea were measured before and after phacoemulsification in senile cataract patients with non-contact corneal endothelial microscope.Corneal endothelial cell density and coefficient of variation(CV)were measured before and 1 month after phacoemulsification in patients with different types of cataract.Results Corneal thickness in the central area of the eyes of senile cataract patients increased 2 hours,4 days and 15 days after operation.The endothelial cell density of senile cataract patients decreased 4 days after operation and remained at a constant level from 15 days to 30 days after operation.The endothelial cell density of lower cornea of senile cataract patients after phacoemulsification were most reduced and compared with the upper and middle corneas.The average decrease of corneal endothelial cell density was the largest in 70~79 years old senile cataract patients after phacoemulsification.Compared with the senile cataract group,the corneal endothelial cell density decreased and the coefficient of variation increased in the degree of myopia complicated cataract group and the diabetic cataract group.Conclusion The apoptosis of corneal cells can produce inhibitor of apoptosis by feedback,and the corneal thickness and endothelial cell density can be maintained at a stable level.
