伴自杀行为和自杀观念抑郁症患者前额叶近红外光图谱分析 被引量:6

Analyzing hemoglobin activation at prefrontal cortex for suicidal behavior and suicidal ideation in depressed patients

摘要 目的通过近红外光光谱分析仪探讨伴自杀行为和自杀观念的抑郁症患者前额叶功能激活的特点。方法选择语义相关的言语流畅性任务作为刺激任务,对23例伴有自杀行为的抑郁症患者(自杀行为组)、28例伴有自杀观念的抑郁症患者(自杀观念组)和26名性别、年龄、受教育年限相匹配的健康对照者(对照组)进行测试,利用FOIRE-3000采集3组受试者前额叶氧合血红蛋白(Oxy-Hb)相对浓度的数据,应用SPSS对数据进行分析。结果自杀行为组和自杀观念组的任务执行结果差异无统计学意义(F=11.196,P>0.05),但均小于对照组(P<0.05)。自杀行为组和自杀观念组Oxy-Hb平均浓度低于对照组,差异主要位于双侧眶额叶皮质和背外侧前额叶区(P<0.05)。结论具有自杀行为和自杀观念的抑郁症患者双侧眶额叶皮质和前额叶背外侧区功能下降。 Objective To explore the characteristics of prefrontal cortex activation by near-infrared spectroscopy(NIRS)in depressed patients who also had suicide ideation or committed suicide.Methods Total 23 patients who suffered major depression disorder(MDD)complicated with suicide behavior(suicide behavior group),28 MDD patients bearing suicide ideation without behavior(suicide ideation group),and 26 healthy controls,matched with the gender,age,and education,were tested by category verbal fluency task(VFT).During the task,52-channel FOIRE-3000 of Shimadzu was used to collect the relative concentration of oxygenate hemoglobin(Oxy-Hb)from all three groups.Results There was no significant difference between suicide behavior group and suicide idea group in their VFT performance(F=11.196,P>0.05),which however were both lower than the healthy control group(P<0.05);the Oxy-Hb exchange of the bilateral orbitofrontal cortex and bilateral dorsolateral prefrontal cortex in suicide behavior group and the suicide idea group were significantly lower than the healthy control group(P<0.05).Conclusion The activity of the prefrontal cortex in MDD patients who had suicide ideation or committed suicide is significantly lower with the most prominent change found in bilateral orbitofrontal cortex and bilateral dorsolateral prefrontal cortex.
