超氧化物歧化酶检测辅助诊断2型糖尿病肾病 被引量:2

摘要 目的探讨超氧化物歧化酶(superoxide dismutase,SOD)检测对2型糖尿病肾病(Type 2 diabetic nephropathy,DN)的辅助诊断价值。方法选取2016年9月至2018年10月间在皖南医学院第一附属弋矶山医院收治的糖尿病患者,采用随机区组设计,将研究对象分为实验组和对照组,实验组选取临床诊断为糖尿病(diabetes mellitus,DM)和糖尿病肾病的患者126例,同期选择82例体检的健康人作为正常对照组。采集糖尿病患者入院24h内的血清标本,统一检测超氧化物歧化酶(SOD)、葡萄糖(GLU)、胱抑素C(CYS-C)、尿素氮(BUN)、肌酐(Cr)。结果GLU、BUN、Cr、CYS-C和SOD在DM组、DN组与健康组的差异有统计学意义(P<0.05);多因素logistic回归分析显示SOD是预测2型糖尿病肾病的独立危险因素(P<0.05);检测SOD对糖尿病肾病的敏感度和特异度分别为74.3%和64.3%,ROC曲线显示SOD的诊断效能AUC为0.751。结论SOD在辅助诊断2型糖尿病肾病方面具有较高的敏感度和特异度,可将SOD检测作为临床早期诊断2型糖尿病肾病的辅助指标。 Objective To investigate the auxiliary diagnostic value of detection SOD in type 2 diabetic nephropathy.Methods The subjects were selected from September 2016 to October 2018 in the first affiliated Yijishan hospital of Anhui Wannan medical college,and divided into experimental group and control group by random block design.126 patients with diabetes mellitus and diabetic nephropathy were selected as the experimental group,and 82 healthy subjects were chosen as the normal control group at the same time.Serum samples were collected within 24 hours after admission,and the level of SOD,GLU,CYS-C,BUN,and Cr were unified detection.The t test,single factor variance analysis and multivariate logistic regression analysis were used to analyze the data.Results There were differences between GLU,BUN,Cr,CYS-C and SOD in the DM,DN and healthy groups(P<0.05).Multivariate logistic regression analysis showed that serum SOD level was an independent risk factor for predicting type 2 diabetic nephropathy(P<0.05).The sensitivity and specificity of detecting SOD,for the diagnosis of DN were 74.3%and 64.3%.The ROC curve showed that the diagnostic efficacy of SOD was 0.751.Conclusion SOD have higher sensitivity and specificity in the auxiliary diagnosis of type 2 diabetic nephropathy,and SOD can be used as an auxiliary index for early clinical diagnosis of type 2 diabetic nephropathy.
