矽肺患者血清miRNA-21、TGF-β1及Smad3的表达及临床意义 被引量:4

摘要 目的探讨矽肺患者血清miRNA-21、TGF-β1及Smad3的表达水平及临床意义。方法选取确诊的矽肺患者共计90例,包含Ⅰ期、Ⅱ期、Ⅲ期患者各30例,为矽肺组;同一采矿环境的同期接尘工人20例,为接尘组;同期参加体检的健康志愿者36例,为对照组。随机选取上述入组对象中4例矽肺患者和4例健康志愿者,抽取血清标本进行miRNAseq高通量测序,了解miRNA-21的相对表达情况。采取实时定量聚合酶链反应(Real-time quantitative polymerase chain reaction, PCR)方法,检测各组对象血清miRNA-21表达水平;采用酶联免疫吸附试验(Enzyme linked immunosorbent assay, ELISA)方法,检测各组对象血清TGF-β1和Smad3水平,探讨三指标之间相关性及与矽肺疾病严重程度的相关性。结果高通量筛选提示,与健康志愿者比较,矽肺患者血清miRNA-21表达量为8.26。矽肺组血清miRNA-21总体表达量为(1.95±0.62),与对照组(0.98±0.08)和接尘组(1.15±0.14)比较,明显升高,差异有统计学意义(均P<0.05)。与对照组相比,接尘组miRNA-21表达量升高,差异有统计学意义(P<0.05)。矽肺组Ⅰ期、Ⅱ期、Ⅲ期各亚组的miRNA-21表达量分别为(1.29±0.14),(2.05±0.49),(2.51±0.38),呈逐步升高,各亚组间差异有统计学意义(均P<0.05)。矽肺组血清TGF-β1总体测值为(521.16±150.68)ng/L,高于对照组(280.03±51.80)ng/L和接尘组(304.00±56.09)ng/L,差异有统计学意义(均P<0.05),接尘组TGF-β1与对照组比较,差异无统计学意义(P>0.05)。矽肺组Ⅰ期、Ⅱ期、Ⅲ期各亚组的TGF-β1表达量分别为(378.00±73.90)ng/L,(507.80±77.91)ng/L和(677.67±106.88)ng/L,呈逐步升高,各亚组间差异有统计学意义(均P<0.05)。矽肺组血清Smad3的总体测值为(32.94±9.31)ng/L,高于对照组(17.39±3.66)ng/L和接尘组(18.98±3.90)ng/L,差异有统计学意义(P<0.05)。矽肺组Ⅰ期、Ⅱ期、Ⅲ期各亚组的Smad3表达量分别为(27.00±4.88)ng/L、(31.44±7.61)ng/L和(40.38±9.39)ng/L,呈逐步� Objective To evaluate the level and clinical value of serum miRNA-21, TGF-β1 and Smad3 in patients with silicosis. Methods It selected 90 patients with silicosis as the silicosis group, including each 30 cases at I, Ⅱ and Ⅲ stage, 20 workers exposed to SiO2 dust during the same period as the exposure group, and 36 healthy volunteers as the control group, respectively. The expression of diverse miRNAs in four silicosis patients and four healthy volunteers were tested by high-throughput sequencing during same period from those admitted patients mentioned above. The level of miRNA-21,TGF-β1 and Smad3 in serum were evaluated by qRT-PCR and ELISA method,respectively. Results High throughput screening found the expression of miRNA-21 was higher in silicosis patients than healthy volunteers with the fold change 8. 26. The level of serum miRNA-21( 1. 95 ± 0. 62),TGF-β1( 521. 16 ± 150. 68) ng/L and Smad3( 32. 94 ± 9. 31) ng/L,were significantly higher in the silicosis group than in the control group [( 0. 98 ± 0. 08),( 280. 03 ± 51. 80) and( 17. 39 ± 3. 66) ] and in the dust-exposure group[( 1. 15 ± 0. 14),( 304. 00 ± 56. 09) and( 18. 98 ± 3. 90) ]( all P < 0. 05),respectively. In the dust-exposed group,the level of miRNA-21 expression was significant higher than that in the control group,but no similar change was found for TGF-β1 or Smad3( P < 0. 05). The level of miRNA-21,TGF-β1 and Smad3 all increased gradually from stageⅠ to stage Ⅲ in silicosis with the significant statistical difference among the three subgroups( all P <0. 05). The level of miRNA-21 were correlated to the level of TGF-β1 and Smad3 in the silicosis group but no similar phenomenon was found among these three indexes at stage Ⅰ,Ⅱ,Ⅲ silicosis subgroups. All of miRNA-21,TGF-β1 and Smad3 were found to have correlation with silicosis stages. Conclusion Serum miRNA-21,TGF-β1 and Smad3 may be biomarkers for auxiliary diagnosis and evaluation the disease severity of silicosis. The level of miRNA-21 may be a new index for dust-ex
