摘要
目的研究无托槽隐形矫治器远中移动磨牙三维方向上的移动量及表达效率。方法选择安氏Ⅰ类和安氏Ⅱ类的成人患者,使用无托槽隐形矫治器推磨牙向远中移动,其中安氏Ⅰ类组9例,患者平均年龄23岁,向远中移动上颌下颌磨牙,安氏Ⅱ类组10例,患者平均年龄24岁,向远中移动上颌磨牙。根据矫治前和推磨牙结束后拍摄的头颅侧位片,计算出磨牙三维方向上的移动线距,角度,远中移动效率,前牙支抗变化情况,将所有数据进行统计分析。结果安氏Ⅱ类组:上颌第一磨牙远中移动表达率为65.86%,第二磨牙远中移动表达率为71.68%;安氏Ⅰ类组,上颌第一磨牙远中移动表达率为73.88%,第二磨牙远中移动表达率为77.73%,下颌第一磨牙远中移动表达率为69.1%,第二磨牙远中移动表达率为75.03%。磨牙远中移动过程中有轻度压低,还存在一定程度的远中倾斜。前牙支抗无明显丧失(P>0.05)。结论利用无托槽隐形矫治器能够有效的远中移动磨牙,但伴有一定程度的磨牙远中倾斜,垂直向上的压低,前牙支抗基本无丧失。
Objective To study the 3 D direction movement and the expression efficiency of molar distalizaiton with bracketless invisible appliance.Methods Adult patients of Angle ClassⅠand Angle ClassⅡwere selected to use bracketless invisible appliance for molar distalization.Among them,group Angle ClassⅠ(9 cases),with the average age of 23 years old,made maxillary and mandibular molar distal movements.Group Angle ClassⅡ(10 cases),with the average age of 24 years old,made maxillary molar distal movement.The lateral cephalometric radiographs were taken before and after the treatment of molars,and movement distance,angle,movement efficiency,anchorage loss of anterior teeth in three dimensional direction were calculated.Statistical analysis of all data was conducted.Results Angle ClassⅡ:The distal maxillary expression rates of the first molar and second molar were 65.86%and 71.68%.Angle ClassⅠ:The distal maxillary expression rates of the first molar and second molar were 73.88%and 77.73%.The distal mandibular expression rates of the first molar and second molar were 69.1%and 75.03%.There was a certain degree of molar distal tilt and vertical upward intruding.There was no significant loss of anchorage in anterior teeth.Conclusion The use of the bracketless invisible appliance is effective in molar distalizatioin,but with a certain degree of molar distal tilt,vertical upward intruding,and the anchorage of anterior teeth is basically intact.
