非洛地平联合曲美他嗪用于冠心病心绞痛患者疗效评价及对冠脉内皮功能和血小板活性的影响 被引量:18

摘要 目的探讨非洛地平联合曲美他嗪用于冠状动脉粥样硬化性心脏病(冠心病)心绞痛的疗效及对冠状动脉(冠脉)内皮功能和血小板活性的影响。方法选择2017年1月至2018年12月于宁海县第一医院接受治疗的98例冠心病心绞痛患者为研究对象,随机分为对照组(n=51)和观察组(n=47),对照组采用曲美他嗪治疗,观察组在对照组基础上联合非洛地平治疗,比较两组疗效,治疗前后心绞痛发作次数、持续时间、西雅图心绞痛(SAQ)量表评分、冠脉内皮功能、血小板活性和不良反应发生情况等。结果观察组总有效率高于对照组(P<0.05)。治疗前,两组患者心绞痛发作次数、持续时间、SAQ量表评分、冠脉内皮功能、血小板活性指标比较差异均无统计学意义(P均>0.05);治疗后,两组患者心绞痛发作次数、持续时间、内皮素-1(ET-1)、血小板活性指标均较治疗前下降,观察组以上指标均低于对照组;两组患者SAQ量表评分、一氧化氮(NO)均较治疗前上升,观察组高于对照组,差异有统计学意义(P<0.05)。两组患者总的不良反应发生率比较无统计学差异(P>0.05)。结论非洛地平联合曲美他嗪可提高冠心病心绞痛的疗效,改善冠脉内皮功能,抑制血小板活性,从而有效控制病情。 Objective To investigate the curative effect of felodipine combined with trimetazidine and its influence on coronary artery endothelial function and platelet activity in patients with coronary heart diseaseangina pectoris(CHD-AP). Methods The patients with CHD-AP(n=98) were chosen from the First Hospital of Ninghai County from Jan. 2017 to Dec. 2018, and were divided randomly into control group(n=51) and observation group(n=47). The control group was treated with trimetazidine and observation group was treated with felodipine combined with trimetazidine. The curative effect, attack times and duration of CHD-AP, scores of Seattle Angina Questionnaire(SAQ), coronary artery endothelial function and platelet activity before and after treatment and incidence of adverse reactions were compared between 2 groups. Results The total effective rate was higher in observation group than that in control grouop(P<0.05). Before treatment, the difference in attack times and duration of CHD-AP, SAQ scores, coronary artery endothelial function and platelet activity had no statistical significance between 2 groups(all P>0.05). After treatment, the attack times and duration of CHD-AP, endothelin-1(ET-1) and platelet activity all decreased and were lower in observation group than those in control group, and SAQ scores and nitric oxide(NO) increased and were higher in observation group than those in control group(P<0.05). The total incidence rate of adverse reactions had no statistical difference between 2 groups(P>0.05). Conclusion Felodipine combined with trimetazidine can promote the curative effect on CHD-AP, improve coronary artery endothelial function, inhibit platelet activity and control effectively the disease.
