摘要
目的旨在分析武进区某三级公立医院实施药品零差率对其经济运营数据的影响。方法从医院财务分析报表和医疗收入及医疗成本明细表中提取2013-2017年经济运行的相关数据,采用描述性分析方法。结果实施药品零差率销售以来,医院总收入和医疗收入仍稳步增加,而药品收入增长幅度不大,年均增长率为6.98%;药品收入占医疗收入比重比从2013年的42%下降至2017年的35.98%,年均增长率为-3.79%;门急诊均次费用、门急诊均次药费分别由255.2、114.0元增加到305.5、135.4元(年均增长率为4.43%、4.42%);出院均次费用从10 929.2元增加到13 248.4元,出院均次药费(元)从4 489.8元下降至4 357.4元(年均增长率为-0.84%)。结论实行药品零差率取得一定成效如药占比的下降及患者均次药费的下降等;进一步完善医院补偿机制;进一步完善医疗服务价格体系;进一步加强医院内部规范化管理。
Objective To analyze the impact on hospital economic operation data by implementing Zero-markup Drug Policy in a tertiary public hospital of Wujin District, Nanjing City. Methods The related hospital economic operation data from 2013 to 2017 were extracted from the hospital financial statement reviews and detailed statements of the medical incomes and costs and analyzed by descriptive analysis method. Results Since the implementation of zero-markup drug sales, the total income and medical income of the hospital were still increased steadily;the drug income showed a little increase trend with an average annual increase of 6.98%. The proportion of drug income in medical income dropped from 42% in 2013 to 35.98% in 2017, which decreased by 3.79% on average. The average outpatient and emergency expense per time and the average outpatient and emergency drug expense per time increased from 255.2, 114.0 yuan to 305.5, 135.4 yuan respectively(with annual increase of 4.43%, 4.42% on average). The average inpatient charge per time increased form 10 929.2 yuan to 13 248.4 yuan;the average inpatient drug expense per time decreased from 4 489.8 yuan to 4 357.4 yuan(with a decrease rate of 0.84%). Conclusions Some effects have been achieved in the implementation of Zero-markup Drug Policy, including the decrease in the percentage of drug expenditure and averaged drug cost per time. It is suggested to further improve the compensation mechanism, the price system of medical services and the standardized management of hospitals.
