摘要
目的分析医药价格改革对公立医院收入结构的影响，并对医药价格改革提出建议。方法收集广东省6家公立医院2010～2014年的相关收入数据，采用比率分析法和描述性统计方法对医院总收入、收支结余及收入结构变化等情况进行分析。结果2010—2014年，6家公立医院总收支均有盈余，但是医疗收支总体呈亏损状态，并且亏损逐年增大，2014年医疗收入亏损达40625．22万元，比2013年增加14．87％。与2010年相比，2014年公立医院医疗收入占总收入的比重上升了19．52％，药品收入占总收入的比重下降了2．52％，其他收入占总收入的比重下降了15．78％，而2010～2014年财政补助收入占总收入的比重均低于3％。结论医药价格改革后公立医院收入结构显著改变；医药价格改革需要政府财政的大力支持，在进一步完善公立医院财政补助政策的同时，改革不合理的医疗服务定价机制并创新药品价格监测方式。
Objective To analyze the influence of medical price reform on revenue structure in public hospitals, and to provide the suggestions for medical price reform. Methods Related revenue data of 6 public hospitals in Guangdong Province from 2010 to 2014 were collected. Ratio analysis and descriptive statistical method were used to analyze hospital gross income, the difference between income and expenditure, and change of revenue structure etc. Results From 2010 to 2014, surplus of gross income over expenditure was achieved in 6 public hospitals, but deficit of medical income over expenditure existed and increased year by year. The deficit of medical income over expenditure reached 406.2522 million yuan in 2014, and 14.87% up on 2013. Compared to the indices in 2010, the proportion of medical income with gross income increased by 19.52%, the proportion of pharmaceutical income with gross income decreased by 2.52%, and the proportion of other incomes with gross income decreased by 15.78% in 2014. The proportions of fiscal subsidy income with gross income from 2010 to 2014 were all less than 3%. Conclusion The revenue structure changes obviously in public hospitals after the medical price reform. The medical price reform requires a strong financial support from government. The policies of financial aid for public hospitals should be further improved, unreasonable medical service pricing mechanisms should be reformed, and the methods for medical price regulation should be innovated.
