摘要
目的探讨半乳糖凝集素-3(Galectin-3)、细胞角蛋白19(CK19)及甲状腺过氧化物酶(TPO)表达在甲状腺肿瘤病理诊断中的应用。方法选取2016年10月—2018年9月期间于广东医科大学附属医院行手术治疗的70例甲状腺肿瘤患者为研究对象,根据WHO甲状腺组织学分型分为甲状腺癌组(n=47)与甲状腺良性病变组(n=23)。所有患者均行免疫组化EnVision法测定Galectin-3、CK19及TPO表达,对比两组Galectin-3、CK19及TPO的阳性表达率,计算Galectin-3、CK19及TPO单独诊断及联合诊断的准确度、灵敏度、特异度。结果甲状腺癌组Galectin-3、CK19阳性表达率较甲状腺良性病变组高,TPO阳性表达率较甲状腺良性病变组低(P<0.05);Galectin-3与CK19单独诊断的灵敏度最高,为100.00%;Galectin-3单独诊断的准确度最高,为94.29%;TPO单独诊断的特异度最高,为95.65%;Galectin-3联合CK19诊断灵敏度最高,为100.00%;Galectin-3联合TPO与Galectin-3、CK19及TPO联合诊断特异度最高,均为100.00%。结论 Galectin-3、CK19及TPO可用于甲状腺肿瘤病理诊断中,且联合诊断可有效提升诊断灵敏度及特异度。
Objective To investigate the expression of galectin-3,cytokeratin 19(CK19)and thyroid peroxidase(TPO)in the pathological diagnosis of thyroid tumors.Methods70 patients with thyroid tumors who underwent surgical treatment in the hospital from October,2016 to September,2018 were divided into thyroid cancer group(n=47)and thyroid benign lesion group(n=23)according to WHO thyroid histological classification.The expression of Galectin-3,CK19 and TPO were measured by immunohistochemical EnVision method.The positive expression rates of Galectin-3,CK19 and TPO were compared between the two groups,and Ga was calculated.Accuracy,sensitivity and specificity of Galectin-3,CK19 and TPO diagnosis alone and in combination were caculated.ResultsThe positive rates of Galectin-3 and CK19 in thyroid cancer group were higher than those in benign thyroid lesion group,and the positive rates of TPO were lower than those in benign thyroid lesion group(P<0.05);the sensitivity of Galectin-3 and CK19 was the highest(100.00%),the accuracy of Galectin-3 was the highest(94.29%)and the specificity of TPO was the highest(95.65%).The diagnostic sensitivity of Galectin-3 combined with CK19 was 100.00%.The diagnostic specificity of Galectin-3 combined with TPO,Galectin-3,CK19 and TPO was 100.00%.ConclusionGalectin-3,CK19 and TPO can be used in pathological diagnosis of thyroid tumors,and combined diagnosis can effectively improve the sensitivity and specificity of diagnosis.
