Expression and its Significance of CK19,Galectin-3,MC and TPO in Thyroid Papillary Micro-carcinoma

摘要 目的:探讨细胞角蛋白19(CK19)、β-半乳糖苷结合蛋白(Galectin-3)、间皮表面微绒毛抗原(MC)及糖基化血红素蛋白(TPO)在甲状腺微小乳头状癌和良性乳头状增生中的表达及其在诊断与鉴别诊断中的意义。方法:采用免疫组化SP法检测34例甲状腺微小乳头状癌与31例良性乳头状增生中CK19、Galectin-3、MC和TPO的表达。结果:CK19、Galectin-3、MC和TPO的阳性表达分别为97.1%,97.1%,94.1%,11.8%;在31例甲状腺良性乳头状增生性病变中CK19、Galectin-3、MC和TPO的阳性表达分别为25.8%、22.6%、3.2%、100.0%。CK19、Galectin-3、MC和TPO在甲状腺微小乳头状癌和良性乳头状增生性病变中的表达差异有显著性(P<0.01)。四种抗体在鉴别诊断中的敏感性分别为97.1%、97.1%、94.1%、88.2%;特异性分别为74.2%、77.4%、96.8%、100.0%;准确度分别为86.2%、87.7%、95.4%、93.8%。结论:CK19、Galectin-3、MC及TPO是诊断甲状腺微小乳头状癌的重要标志。联合检测四种抗体在甲状腺微小乳头状癌与甲状腺良性乳头状增生的鉴别诊断中具有重要价值。 Objective:To observe the expression and differential diagnosis of CK19, Galectin -3, MC and TPO in papillary thyroid micro -carcinoma and papillary carcinoma with benign lesions. Methods:expression of CK19, Galectin -3, MC and TPO were measured by SP immunohistochemical technique in 34 cases of papillary thyroid micro- carcinoma and 31 cases of papillary carcinoma with benign lesions. Results: The positive rates of CK19, Galectin- 3, MC and TPO were 97.1% ,97.1% ,94.1% and 11.8% in patients with papillary thyroid micro - carcinoma, respectively, and they were 25.8% ,22.6% ,3.2% and 100% in papillary carcinoma with benign lesions respectively. There was a significant difference between CKI9, Galectin- 3, MC and TPO in papillary thyroid micro the both grouPS (P 〈 0.01 ). The sensitivity of -carcinoma were 97. 1%, 97. 1% ,94. l% and 88.2%respectively, and the specificity was 74.2% ,77.4% ,96. 8% and 100% ; the accuracy was 86.2%, 87.7% ,95.4% and 93.8%, respectively. Conclusions:CKl9 Galectin- 3 MC and TPO are the significant signs in diagnosis of papillary thyroid micro - carcinoma. And combination of the four antibodies hold great value for differential diagnosis between papillary thyroid micro -carcinoma and papillary carcinoma with benign lesions.
